About the ACT Team

Assertive Community Treatment (ACT) is an evidencebased clientcentered service model that provides comprehensive communitybased mental health treatment and support. We believe in delivering care where its needed mostout in the community where our clients live work and interact.

ACT provides highly integrated services from a multidisciplinary team of professionals including mental health clinicians medical staff and substance use experts. We work with adults (ages 18 and over) who have Serious and Persistent Mental Illness (SPMI) and often experience cooccurring substance use disorders. These individuals may face significant barriers to accessing traditional treatment settings. ACTs goal is to support clients by bringing services directly to themwhether its in their homes shelters or other familiar settingshelping them integrate into their communities and lead independent lives.

Job Summary

As a Qualified Mental Health Professional (QMHP) on the ACT Team you will play a pivotal role in providing recoveryoriented crisis and therapeutic case management services to individuals with severe mental illness and cooccurring substance use disorders. You will offer services both in individual and group settings using evidencebased clinical practices harm reduction strategies and traumainformed care to help clients move toward stability and independence.

This position is key to delivering intensive communitybased mental health services. Your role will include:

  • Mental health assessments and service planning tailored to individual client needs
  • Crisis intervention and management for clients experiencing acute episodes
  • Community outreach to engage clients where they live work and socialize
  • Collaboration with a multidisciplinary team of professionals to deliver integrated services
  • Evidencebased therapeutic techniques to address mental health challenges
  • Cultural competence in working with diverse populations ensuring services are inclusive and accessible to all

MINIMUM QUALIFICATIONS
The following minimum qualifications are REQUIREDfor this position:
QMHP Health Care Professional 2 level (HCP42:
  • Masters degree from an accredited college or university in Counseling Social Work Psychology or closely related field; AND
  • One yearof health experience providing mental health services;AND
  • Must be registered as a Qualified Mental Health Professional (QMHP) with Mental Health & Addiction Certification Board of Oregon (MHACBO) or be able to obtain certification within 6 months of hire.

QMHP Health Care Professional 3 level (HCP43:
  • Masters degree from an accredited college or university in Counseling Social Work Psychology or closely related field; AND
  • Two yearsof health experience providing mental health services; AND
  • PsyD LCSW LPC LMFT or equivalent licensure.

A Masters degree is required with the exception of a Bachelors degree in Nursing or Occupational Therapy. Please note candidates must have at least 6 months of actual work experience to be considered as equivalent.


Special Requirements
  • Hiring is contingent upon the successful completion of a background check.
  • Valid Oregon drivers license. This position requires driving and may require use of a personal vehicle.

Ideal Candidate
  • PsyD LCSW LPC LMFT or equivalent licensure
  • CADC obtained
