We are an 85bed active living community offering residential care secured dementia care skilled rehabilitation and longterm care nursing services. We provide medical care 24hour personal care private and shared rooms a full calendar of events and activities laundry housekeeping and delicious meals. Our goal is to deliver compassionate care to our residents in an affordable allinclusive environment.

What Youll Do

Evaluate patients and develop a comprehensive treatment plan to restore patients to their maximum functional potential according to acceptable principles and practices of physical therapy.

Direct therapeutic intervention for the restoration of patients functioning to prevent disability following injury disease or physical disability.

Provide care to patients in a setting of the long term care facility.

Assist patients to reach their maximum performance and level of functioning while learning to live within the limits of their capabilities.

Collaborate as a member of the healthcare team for the provision of comprehensive patient care.

Maintain consistent with training and competency review including regulatory requirements such as JCAHO (if applicable) and state and federal regulations and standards.

Skills and Attributes for Success

High level of professionalism.

Ability to identify and respectfully address psychosocial cultural ethnic and religious/spiritual needs of patients and their families.

Ability to meet current documentation standards and policies in a timely fashion.

Ability to read and communicate effectively in English.

Strong written and verbal skills.

Basic computer skills.

Requirements and Qualifications for the Position

Current State Physical Therapy Licensure.

Current BCLS certification.
