Job Summary
The Physical Therapist (PT) delivers exceptional patientcentered care by managing the entire therapeutic process for a specific group of patients in collaboration with physicians and the healthcare team. The PT conducts patient assessments and crafts tailored care plans aimed at achieving goals engaging patients and enhancing their overall experience. The PT administers or oversees most of the patients physical therapy to assist them throughout their care journey.
Job Responsibilities
- Conduct evaluations and deliver treatment interventions within the legal and ethical scope of practice.
- Assess therapeutic rehabilitative and functional status and contribute to developing the comprehensive care plan.
- Provide effective and skilled therapeutic care in patients homes employing techniques that lead to changes in condition or function in line with the physician s care plan.
- Educate patients families and caregivers on using therapies exercises therapeutic devices and prosthetic appliances outside therapy sessions.
- Prioritize the caseload determine daily staffing requirements and manage assignments and handoffs.
Qualifications
- Basic Cardiac Life Support: Required within 30 days of starting employment
- Physical Therapy License: Required
- Education: Bachelor s Degree
