Home Health Physical Therapist (PT) $105000$150000

Essential Responsibilities:
- Perform comprehensive patient evaluations and physical assessments

- Track and document patient progress using electronic health records (EHR)

Required Qualifications:
- Masters degree in Physical Therapy from an accredited program
- Active state Physical Therapy license
- 1 year of home health PT experience
- Current CPR certification
- Valid drivers license with reliable transportation
- Strong communication and interpersonal skills
- Proficiency in EHR documentation
