Position: Community Support Professional (CSP)
Compensation Range: $15.70$18.95
Hours: Flexible/Variable Full time part time and per diem schedules available.
The Community Support Professional will provide inhome and communitybased habilitation services to individuals within the Community Habilitation program. The Community Support Professional must be able to support individuals in exploring activities that allow them to develop life skills and achieve their personal goals. Additional responsibilities include but are not limited to:
*Being flexible with scheduling based on individuals needs
*Selfmotivated creative and willing to work autonomously
*Providing transportation when necessary to/from individuals home and communitybased activities appointments etc.
*Documenting services provided as outlined in individuals service plan
Minimum Qualifications:
* High school diploma or GED (in lieu of diploma/equivalent a passing score as outlined on standardized tests may be accepted)
* Valid NYS drivers license which meets agency guidelines
* Basic computer skills required access to a mobile device necessary and access to a home computer beneficial
* Previous experience in ID/DD field preferred
