Home Health Physical Therapist FullTime Position

- Deliver patient and caregiver education on rehabilitation techniques and injury prevention
- Document patient progress and maintain detailed electronic health records
- Collaborate with interdisciplinary healthcare teams to ensure optimal patient outcomes
- Participate in care plan reviews and updates as patient conditions evolve
Required Qualifications:
- Masters degree in Physical Therapy from an accredited program
- Current state Physical Therapy license in good standing
- Minimum 1 year of home health physical therapy experience
- Valid drivers license and reliable transportation
- Current CPR certification
- Strong interpersonal and communication skills
- Proficiency in electronic documentation systems
