Home Health Physical Therapist PatientCentered Rehabilitation Specialist
Were seeking a dedicated and experienced Home Health Physical Therapist to join our growing healthcare team. In this role youll provide specialized rehabilitation services to patients in their homes helping them achieve optimal physical function and independence. This position offers a competitive salary range of $105000 $150000 annually.
Essential Responsibilities:
- Perform comprehensive physical assessments and evaluate patient mobility strength and functional capabilities
- Develop evidencebased treatment plans aligned with patientspecific rehabilitation goals
- Execute therapeutic interventions including manual therapy gait training and functional exercise programs
- Provide patient and caregiver education on rehabilitation techniques fall prevention and home safety
- Document treatment progress using electronic health records (EHR) systems
- Coordinate care plans with interdisciplinary healthcare teams
- Maintain compliance with Medicare HIPAA and other healthcare regulations
Required Qualifications:
- Masters degree in Physical Therapy (DPT preferred)
- Current state Physical Therapy license in good standing
- Minimum 1 year of home health care experience
- Valid CPR certification
- Current drivers license and reliable transportation
- Strong understanding of Medicare guidelines and documentation requirements
- Excellent interpersonal and clinical decisionmaking skills
