Who we are: La Clnica de La Raza is a communitybased health center committed to providing culturally appropriate highquality and accessible health care to the diverse communities of the San Francisco Bay Area. We have spent over 40 years advocating for and creating a health home for the many that have been denied access to care. As health care activists we are dedicated to making sure individuals who do not have health coverage get the same level of quality healthcare as those who have it. From our genesis as a community health center in 1971 we understood that we cannot heal our communities without also addressing the economic and social factors that affect health. Many times this requires that we go outside and provide services in other settings to build the bridge that links individuals to our health center for preventive and primary care. Over 90000 individuals come to La Clnica because it is a welcoming place that addresses the whole person coordinating and connecting them to a broad network of services to improve and maintain their health and wellbeing. While we are still known for our activism and spirit of social justice we are also proud to have grown into a sophisticated provider of primary health care services with 35 sites across Alameda Contra Costa and Solano counties.

Learn more about each of the three loan repayment programs below.

1. NHSC Loan Repayment Program awards up to $75000 in exchange for a twoyear commitment to provide primary medical dental or mental/behavioral health care at approved sites in highneed underserved areas. All other health professionals can receive up to $50000 in awards. NHSC LRP recipients are eligible to extend their service and potentially pay off all of their educational debt with continuation contracts.
2. NHSC Substance Use Disorder Workforce Loan Repayment Program provides up to $75000 in exchange for a threeyear commitment from health care professionals who provide substance use disorder (SUD) treatment services at NHSCapproved SUD treatment facilities.
3. NHSC Rural Community Loan Repayment Program Providers in rural communities may apply to the NHSC Rural Community LRP which awards up to $100000 for three years of service.

Major Areas of Responsibility include but are not limited to:

Healthcare Delivery

• Deliver evidenced based care that supports our aim for improved health outcomes exceptional patient experience and efficient use of resources.
• Perform routine acute urgent preventive curative and rehabilitative medical care to patients and determine appropriate regimen.
• Prescribe medications treatments or therapies and order medical diagnostic or clinical tests
• Explain medical procedures or test results to patients or family members
• Analyze test data or images to inform diagnosis or treatment.

Patient Experience

• Deliver well and chronic disease care with cultural humility to a diverse patient population of all ages.
• Have flexibility in meeting patients needs and participating in the call pool and rotational Saturday clinics.
• Develop a positive rapport with patients and families to foster the physician/patient relationship.

Collaboration & Mentorship

• Serve as a member of the crossfunctional team of providers consulting with midlevels on the assessment management and treatment of complicated cases chart review cosigning and case conferences.
• Suggest improvements for clinic flow patient scheduling and service delivery and collaborate professionally with interdisciplinary team members including providers health educators medical assistants registered nurses and case managers.
• Provide direction to medical assistants regarding patient care as needed.

Minimum Job Requirements

Knowledge

• Knowledge and experience with Electronic Health Record programs such as NextGen or Epic.
• Knowledge of the information and techniques needed to diagnose and treat human injuries diseases and deformities. This includes symptoms treatment alternatives drug properties and interactions and preventive healthcare measures
• High degree of interpersonal competence with both staff and patients and thorough knowledge of evidence based care
• Good time management skills and evidence based approaches to medical care
• Cultural competence working in diverse/low income communities

Abilities

• Ability to deal courteously and effectively complex patient profiles and maintain good working relationships with other staff
• Ability to work courteously and effectively as a team member to support the quality/efficiency standards established for the clinic; work independently; exercise good judgment; spell correctly; communicate effectively orally and in writing make and thorough manner with speed and accuracy with minimal supervision
• Ability and willingness to work under pressure and as part of an interdisciplinary team
• Demonstrate flexibility and ability to accept changes gracefully
• Ability to work across cultures and demonstrate support of diversity equity and inclusion.
• Ability to keep accurate clear and legible records of patient information
• English/Spanish proficiency preferred but not required

Experience and Other Certifications

• MD/DO degree from a recognized University one to two years of experience working in primary care clinic setting
• Requires board certification in Family Practice. In the absence of a board certification candidate must be board eligible and become board certified within 2 years of employment
• Requires valid California license and DEA
• Must hold a current BLS (Basic Life Support) Certificate. You will have 30day grace period to obtain your certificate from the date of hire.
