ABOUT US

The WinstonSalem Childrens Developmental Services Agency (WS CDSA) is the local lead agency for the NC InfantToddler Program (NC ITP) a federally mandated program (Part C of the Individuals with Disabilities Education Act) serving children with/at risk for developmental disabilities and their families. Information on the NC ITP is available at SUMMARY

Deliver quality early intervention service coordination (targeted case management) to infants and toddlers ages 0 to 3 years and their families. Ensure that infants and toddlers referred and/or enrolled in the NC InfantToddler Program (NC ITP) receive service coordination provided in accordance with and in compliance with Federal and State regulations ensuring parents rights in the program. Provide targeted case management resources and referrals to appropriate community programs as needed within the sixcounty catchment area WS CDSA. Meet NC ITP mandated timelines and documentation for enrollment individualized family services plan (IFSP) intervention services as needed transition planning and transition upon the childs third birthday.

SKILLS AND QUALIFICATIONS

ESSENTIAL FUNCTIONS

  1. Provide targeted case management/service coordination based on compliance with federal IDEA part C and NC ITP guidance governing infanttoddler early intervention.
  2. Ensure family procedural safeguards meeting federal and state required timelines compliance indicators and paperwork for all service coordination activities.
  3. Responsible for explaining NC ITP early intervention to families and ensuring parents rights for eligibility the childs developmental strengths needs and services within NC ITP. Explain enrollment based on eligibility status to parents referral sources and community professionals as needed.
  4. Responsible for gathering information from the family beginning a familydirected identification of family needs strengths priorities concerns and resources.
  5. Participates as a member of a team in conducting developmental evaluations or child assessments determining program eligibility and preparing developing monitoring and revising Individualized Family Service Plans (IFSP).
  6. Assists families in accessing other services and resources as needed. This includes referrals and arranging service provision with followup as needed. Work with other CDSA staff and/or network service providers to meet identified child and family needs/outcomes.
  7. Assists families when child reaches two years of age to prepare for transitioning from the NC Early Intervention Program when the child turns three.
  8. Performs other related duties as assigned and assures that recordkeeping and documentation at all times is compliance with NC ITP WS CDSA and WFBH program requirements.

WORK ENVIRONMENT

Clinic and office settings as well as individual homes and community settings for each child on Developmental Specialists caseload.
