As an Advanced Practice Clinician (APC) you will be part of a transformative program designed to provide personalized care and support to our patients including those with complex and chronic needs all in the comfort of their own homes.**Please note this job posting represents a future opportunity and/or an upcoming class**
The APC will ensure the delivery of highquality clinical and homebased patient care while promoting optimal health outcomes and providing costeffective solutions that adhere to Alignment Healths policies as well as state and federal regulations.
Youll play a vital role in assessing developing and coordinating care options tailored to the needs of our members who are typically 65 and older. With home and virtual visits ranging from 45 to 60 minutes youll have the opportunity to provide focused compassionate care whether its an initial visit an annual checkup or when additional support is needed to address medical and social challenges.
Requirements:
Experience:
Required: None
Preferred: One 1 year of prior clinical or home care experience. Previous EMR experience preferred. Experience in care of older adult (geriatric) patients preferred
Education:
Masters degree from an accredited NP Program or PA program
Licensure:
Required: Active California state Nurse Practitioner or Physician Assistant license
Active RN License and Furnishing number (Must upon hire) NP only
Active Nurse Practitioner Board Certification/Physician Assistant Certification
NPI Number DEA Valid BLS
Valid California driver license and current automobile insurance
