Job Description

Responsibilities & Requirements

This position will provide high quality health navigation and patient education via telephone for qualified patients who are part of a grant from the Substance Abuse Mental Health Administration (SAMHSA). Staff will build and maintain organizational relationships document interactions assist with data collection identify and share communitybased resources and other grantrelated duties as assigned. Services provided through this project are delivered via phone/telehealth.

Because this position is funded 100 by the grant this individual can only perform work related to the grant project.

IMPORTANT: This is a timelimited grantpaid position. Funding from the SAMHSA is for the project period: 9/30/24 to 9/29/27. This position cannot be guaranteed after that time.

Education

  • HS Diploma or GED required.
  • Completion of Community Health Worker training/educational program.

Work Experience

  • Experience in communitybased setting as community health worker or similar position.
  • New graduates of the Community Health Worker Training Program in Kettering College may be accepted with a recommendation from their instructor.

Skills

  • The ability to educate people from various backgrounds using strong verbal communication and active listening skills.
  • Detail oriented especially with patient documentation and tracking.
  • The ability to remain humble and deliver programming in a culturally appropriate way. Must value diversity.
  • Must be willing to learn how to use the electronic health record (Epic) and keep excellent documentation in that system as needed. Epic experience preferred.
  • Proficiency in Microsoft Office applications required.
  • Must work well with the project and grant management teams.
  • Highly motivated and can work independently.

Essential Functions

  • Via telephone contact provide high quality health navigation and patient education for qualified patients who are part of the grantfunded project by assisting them with access to medical services health insurance substance use disorder treatment and other resources and services to improve health outcomes and provide education about pain management. This is a telephonic position with care provided via telephone primarily in the office setting.
  • Following processes and standards established for delivering telephonic care for patients served by the grant.
  • Assure all interactions are documented appropriately.
  • Attend internal and external grant stakeholder meetings.
  • Assure any data needed for grant reporting is collected accurately and given to the Project Manager on time.
  • Assist with report writing when needed.
  • Collaborate with grant project staff to create additional collaborative opportunities with external organizations.
  • Build and maintain professional relationships with communitybased organizations.
  • Ability to deliver services in a patientcentered culturally competent manner.
  • Stay informed about current trends and developments around SUD substance use and pain management.
  • Some local travel may be required. Travel to attend an annual conference which may be instate is encouraged.
  • Collaborate with the Project Manager for purchasing etc.
  • Collaborate with Project Manager and grants management team to assure activities comply with federal grant rules.
  • Maintain timely Time and Effort reporting.
  • Other grantrelated duties as assigned.

Preferred Qualifications

  • Life experience with substance use disorder preferred. (Can be self a family member or a close friend.
  • Certification preferred but not required.
