Job Description Summary
The Telehealth Behavioral Health Care Manager (BHCM) reports to the Collaborative Care Manager in the Center for Telehealth. This position serves as a BHCM for a telehealthenabled collaborative care program supporting patients in rural primary care sites. The BHCM supports patient enrollment in program administers regular behavioral health assessments provides brief intervention motivational interviewing and case management as appropriate all remotely via telehealth. The BHCM is responsible for the coordination of behavioral health treatment with the primary care providers (PCP) at rural sites and proactively communicates patient needs and treatment recommendations to the PCP. The BHCM will also engage in weekly caseload review and consultation with the psychiatric consultant. Additionally the BHCM will support training and implementation of this program at rural clinic sites and serve as a key member of the pilot grant research team.Entity

Additional Job Description
Masters degree from an accredited college/university required in social work psychology counseling or social welfare program plus 12 years of related psychiatric/behavioral health case management experience OR Bachelors degree in social work nursing psychology counseling or social welfare program plus 35 years of related psychiatric/behavioral health case management experience required. Clinical licensure preferred. Critical thinking skills and strong interpersonal skills essential. Excellent organizational and time management abilities and able to work effectively within a multidisciplinary team. Skilled with patient interaction problem solving navigation and listening. This position has the opportunity to work primarily remotely in SC with occasional travel to primary care sites and the MUSC Center for Telehealth in Charleston.
