This is a full time position.
Duties
Participate as a member of the multidisciplinary Assertive Community Treatment (ACT) Team providing comprehensive communitybased rehabilitative mental health treatment including case management and coordination of services to Southeast Asian adults with a severe and persistent mental illness. Initiate and/or coordinate care with other providers as required. Maintain an understanding of current community resources and supports available to clients and refer them or support them in obtaining services as needed. Serve as mandated reporter for child or vulnerable adult neglect and abuse. Consult and work with community agencies and families to maintain needed resources and coordinate services in the treatment process. Assure client has contact with his/her psychiatrist and follow up to monitor that client is taking prescribed medications.
Complete functional assessments treatment plans IDDT EOM risk assessment or other screening as required. Ensure accurate and timely completion of progress notes. Engage with individual clients and families. Participate in regular team meetings. Develop professional relationships and partnerships with other community service providers. Provide community engagement or program outreach as required. Establish and/or selfhelp and support groups for team clients and their families. Offer training and education to help clients learn the necessities of daily living including selfcare money management social skills development use of public transportation and securing employment.

Job Qualifications
Bachelors degree in a Behavioral Science or related field required. Experience may not be exchanged for education. Minimum 1 year experience in family outreach support care coordination counseling or community mental health. Must meet mental health practitioner requirements. Must be bilingual in English and one of the following languages: Hmong or Karen. Valid Drivers License auto insurance and reliable vehicle required.
