Job Description
- Provide assistance with light housekeeping needs that are directly related to the health and wellbeing of the eligible individual i.e. bedmaking dusting vacuuming etc.
- Provide assistance with accessing community resources/shopping/appointments.
- Accurately complete and submit Daily Activity Logs and Contact Summaries in a timely manner.
- Adhere to safe transportation procedures.
- Follow appropriate protocol for completion of I/A and Serious Incident reports.
- Ability to work a flexible schedule based on the familys needs.
- Attend trainings/inservices as requested and adhere to requests for personnel update information in a timely manner.
- Adhere to Activity Plan and/or family instructions regarding personal care of the eligible individual.
- Keep supervisor abreast of pertinent information relevant to the provision of services.
- Follow all safety policies and adhere to all workers compensation program guidelines
OTHER FUNCTIONS:
Perform other functions and tasks as assigned.
EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
High School Diploma or equivalent required. Must be at least 21 years of age.
LICENSES/DESIGNATIONS/CERTIFICATIONS:
Must have a valid drivers license.
