Job Description

The Diabetes/Patient Educator Maternal Fetal Medicine will provide comprehensive high quality nutritional care to patients through assessments diet counseling and diabetes support and education programs.

JOB DUTIES AND RESPONSIBILITIES:

  • Collaboratively develops education goals learning objectives and an individualized plan for education content and teaching methods for diabetes and pregnancy program participants

  • Collects diabetes and pregnancy program participant assessment data using the ADCES framework in a collaborative and ongoing manner.

  • Assesses patients blood glucose levels and develops a plan of care in collaboration with the Perinatologist and Perinatal Nurse Practitioner. Is familiar with and utilizes the Perinatal Diabetes and Pregnancy management protocols

  • Utilizes fetal ultrasound data to assist in blood glucose evaluation. Is familiar with protocols for diagnostic and laboratory testing for the treatment of diabetes and pregnancy. Familiar with the ADA Standards of Care for the Patient with Diabetes

  • Provides education interventions that utilize primarily interactive collaborative skillbased training methods and maximizes the use of interactive training methods

  • Collaboratively develops an individualized followup plan with each program participant

  • Evaluates the effectiveness of the education provided by measuring attainment of learning objectives

  • Conducts ongoing and followup assessments upon completion of diabetes and pregnancy program; utilizes the outcome measures from the AADE7 Continuum of Outcomes Framework

  • Documents assessment data educational plan services provided and evaluation results in each participants education records

  • Utilizes a team approach to provide services collaborates and communicates with team members when needed

PHYSICAL AND SENSORY REQUIREMENTS:

Sit up to 4 hours per day; 2 hours at a time. Stand for up to 6 hours per day; 3 hours at a time. Walk 6 hours per day; 20 minutes at a time. Frequently use hands and fingers to manipulate objects and for computer use. Rare lifting of objects greater than 50 pounds. Occasional transport of patients weighing up to 300 pounds via wheelchair. Must have the ability for hearing as it relates to normal high and low frequency. Must have the ability for seeing as it relates to general vision near vision color vision peripheral vision and depth perception. Occasionally stoop bend or reach

beyond shoulder level. Occasionally squats crouches kneels and climbs. Must have hand/eye coordination.

EDUCATION:

Registered Professional Nurse or Registered Dietician with a Bachelor of Science degree and a current license to practice in the state of Pennsylvania. Certification as a Diabetes Educator preferred. Eligibility for certification may be considered. ADA/CDCES certification as a Diabetes Educator within 2 years.

TRAINING AND EXPERIENCE:

A Registered Nurse or Registered Dietitian who is also a certified diabetes educator or who is eligible to become a certified diabetes educator. ADA/CDCES certification as a Diabetes Educator within 2 years. Two years of diabetes and pregnancy education preferred. Experience with insulin pump therapy preferred.
