SUMMARY: Provides Public Health nursing services and education in clinic and home settings to clients across the age spectrum. Assists Public Health Nurse Manager in the coordination of programs activities and goals with limited budgetary responsibilities.
PRIMARY DUTIES AND RESPONSIBILITIES:

Provides nursing services in clinic and home settings making referrals to other resources as needed and following up as needed.
Performs nursing assessment diagnosis treatment and evaluation of clients across the age continuum.
Teaches individuals and groups in health promotion and disease prevention.
Creates charts and documents recording details of client visits; manages personal case load and follows documentation of other staff engaged with clients in program coordinated; maintains file and filing system.
Assists in the development and delivery of annual performance reviews for assigned staff; schedules and assigns tasks; evaluates work performance and works with staff to reach goals and correct deficiencies; and ensures adherence to established policies and procedures.
Performs administrative duties include data collection and entry; timely response to phone calls and emails; coordination of referrals received from outside entities.
Refers clients to other community resources; works with other providers to reduce barriers to care for the community.
Coordinates specific program; assists with implementing and meeting program grant requirements and program goals; oversees and maintains program quality assurance documentation systems inventory of and client billing for durable and consumable client resources; compiles data and prepares quarterly grant report.
Plans coordinates and oversees employee trainings and orientations.
Participates in health clinics and educational programs with clients as required.
Participates in team responses as required for epidemiological and allhazards situations.
Supports the relationship between the Teton County staff and the general public by demonstrating courteous and cooperative behavior when interacting with visitors and County staff; maintains confidentiality of workrelated issues and County information; performs other duties as required or assigned.
MINIMUM QUALIFICATIONS:
Education and Experience:
Associates Degree in Nursing
3 years of experience in public health nursing programs or related field
Required Licenses and Certifications:
Must possess a valid Wyoming/Idaho Drivers License and maintain an acceptable driving record.
Must be licensed as a Registered Nurse in the State of Wyoming.
Cardio Pulmonary Resuscitation (CPR) Certification.
Required Knowledge of:
Federal state and local laws pertaining to health care and public health.
Health Insurance Portability and Accountability Act (HIPAA) laws and regulations.
Practices principles and methods of professional nursing.
Safety rules and regulations occupational hazards and safety precautions.
Principles and procedures of recordkeeping reporting and retention.
Required Skills in:
Operating a personal computer utilizing standard software and some specialized software; operating standard office equipment.
Providing professional nursing and health care services.
Establishing and maintaining cooperative working relationships with coworkers and other individuals in contact during the course of work; communicating clearly and concisely.
Planning and administering programs and program goals.
Physical Demands/Work Environment:
Work is performed in County clinics as well as in patient homes; may be exposed to adverse conditions and weather during home visits. This position has an extreme exposure risk to biohazards associated with blood borne and airborne pathogens; the use of proper protective equipment and clothing is required. Inoculation may be required.
