Job Description
MondayFriday 8am5pm
Assists and counsels clients and families with financial social and emotional concerns. Hugh Chatham Health at Home (formerly Yadkin Valley Home Health) proudly serves Alexander Alleghany Ashe Davie Forsyth Iredell Stokes Surry Wilkes and Yadkin counties in NC.
Education and Formal Training: Masters degree in social work from an accredited school of social work.
Work Experience: Minimum of one year experience in social work preferably in a home care agency or a hospital outpatient clinic rehabilitation center or mental health program.
Knowledge Skills and Abilities Required: Demonstrates a high level of mental and emotional tolerance and even temperament when dealing with ill people; uses tact sensitivity sound judgment and a professional attitude when relating with patients families and coworkers at all times. Actively functions as a patient advocate. Must develop knowledge of current home care emergency policies and procedures fire safety disaster and infection control policies.
