Job Duties:
Works under direction of RN & requires substantial knowledge of skilled licensed practical nursing procedures in order to provide care for residents. Administer medication & perform treatments prescribed by the Physician. Administer controlled narcotics & noncontrolled medications in oral rectal or injectable form (i.e. intramuscular & subcutaneous injections only). Do med awareness & self med programs as written. Provide instructions to families for pass meds. Maintain supplies in medication rooms & treatment rooms of assigned areas & reorder meds as needed. Perform skilled practical nursing procedures or treatments (i.e. sterile dressings administer oxygen insert catheters reinsert gastrostomy tubes suctioning irrigations give enemas s collect specimens for diagnostic purposes etc.. Monitor environment in the areas of health & safety reports &/or remediate problems as appropriate. Assist in emergencies (i.e. first aide CPR psychotropic emergencies etc. Access emergency box for necessary medications/narcotics check in medications from pharmacy deliveries & maintain proper record keeping. Deliver medication to appropriate cottage med carts. Mandatory overtime is required of this position.
Participate in change of shift report & check individuals reported as being ill or injured & document appropriately. Make rounds in assigned areas & observe & document resident response to treatment & other significant activities. Observe document & report individuals conditions (i.e. medication reactions treatment responses individual behavior & progress) &/or maintain related reports & records (i.e. unusual incident reports patient records medication reports restraint records & logs). Provide general nursing care direct care &/or programming assistance in areas of personal hygiene grooming eating toileting mobility & behavior etc. Transcribe & implement physicians orders & start AIR forms as appropriate. Attend inservices workshops meetings & seminars. Assume responsibility for own professional conduct & competence. Contacts family/guardians with necessary medical information.
Coordinate with Residential Care Supervisor & RN in providing health care to residents. Provide input into Health Assessment & Nursing Care Plan of residents within assigned areas. Coordinate & gather information into IPPs under RN direction. Assist RN & Residential Care Supervisor in orientation of new employees as appropriate. Communicate significant information verbally & in writing to immediate supervisor & other team members as directed by SODC Directive & Nursing Policy & Procedure. Other related duties including but not limited to: monthly weighs of residents as ordered & document. Under the direction of the RN Supervisor help with oneonone staffing & provide hospital coverage for individuals as directed by RN Supervisor. Assist with recreational activities. Complete paperwork on scheduled primary assignment (i.e. quarterly summaries IPPs medical appointments PO sheets MARS chart audits etc. Complete required immunizations/PPDs on assigned individuals. Under the direction of the RN gather necessary information on quarterly physical inspections & Moses Scales for assigned individuals. Communicate medical information to parents/guardians when appropriate. Monitor healthcare charts on cottage to ensure proper followup continuity of health care. Verify monthly physicians orders & MARs. Change over monthly POs and MARs on last day of month as assigned.
