Blue Ridge Health is currently seeking a Clinical Community Health Worker to be part of our Community Outreach Team.

What Youll Do:

The Clinical Community Health Worker is an integral member of the interdisciplinary health team. The clinical outreach specialist can be a Medical Assistant LPN or RN and is responsible for reaching out to vulnerable populations in the community completing basic health screenings point of care testing patient referral and followup as well as case management. Responsibilities include:

  • In conjunction with a peer support specialist goes out into the community to engage and provide health information and services to individuals within the substance use community.
  • Provides basic health screenings and point of care testing out in the field. Refers individuals to healthcare providers and community resources as needed.
  • Goes out into the community to connect and engage with individuals experiencing homelessness and/or substance use and distributes basic supplies refers to community resources provides transportation as needed and provides case management.
  • Builds rapport with community individuals and looks for opportunities to interact with them in order to help meet their current needs.
  • Helps plan organize and staffs community health education and screening events.
  • Occasional evening and weekends may be required.
  • Takes basic vital signs and becomes competent in conducting basic health screenings
  • Serves as ambassador for BRH promoting our services to other agencies employers
    community members and potential clients.
  • Engages with assists clinical staff as appropriate/directed functioning as a member of the clinical team.

What Were Looking For:

  • Active licensure as a Medical Assistant License Practical Nurse or Registered Nurse is required.
  • Lived experience in homelessness and/or migrant agriculture work is highly preferred.
  • High school diploma required.
  • Valid North Carolina drivers license; eligible for coverage under BRH automobile insurance policy
