Description

About the role

The Vulnerable Youth Case Manager provides tailored case management support for our young Aboriginal and Torres Strait Islander people. The Vulnerable Youth Case Manager role supports young people and their families across the Logan and Brisbane region. The role coordinates to a range of internal and external services while delivering targeted case managed supports and interventions. The role is heavily outreach focused helping young people and their families to receive the right service at the right time in the right location.

Duties

  • Effectively engage clients through coldcall homebased visits. Turn initial service offerings home visits into consent by delivering motivational interviewing and positive reinforcement practice interventions.
  • Build strong relationships with clients through effective microcounselling techniques (body language voice tone active listening) and delivering unconditional positive regard in removing shame or blame perceptions of clientbeliefs.
  • Deliver immediate safety assessment and safety responses documented and approved by lineManager in instances of domestic and family violence selfharm/mental health and child abuse.
  • Lead tough conversations with clients outlining nonnegotiables of statutory bodies (Child Safety Police Courts Youth Justice etc.. Build change readiness of clients through motivational interviewing strategies and positive reinforcement.
  • Build client agency selfefficacy and facilitate change through client empowerment while actively discouraging client dependence on supports.
  • Deliver positive reinforcement and continually highlight client strengths and successes to shift/maintain a positive immediate outlook.
  • Deliver case managed services according to the program model of care ensuring minimum case load requirements are maintained to unsure equitable access to services for community.
  • Maintain accurate and detailed case notes of all client interactions ensuring case notes are written from the perspective of clients in their voice.

  • Starting Salary of $73358 per annum (depending on qualification and experience).

Skills And Experiences

COVID19 Requirements

  • Immunisation history showing you have received two doses of a Covid 19 vaccine is encouraged.

  • Current Diploma or actively studying towards Diploma in Social Work Human Services Social Science Health Allied Health Youth Work Community Services Family Intervention or 2 years relevant experience in field.
  • Sound case management skills including grammatical writing ability personal organisation and time management and of services according to the model of care phases of work.
  • Demonstrated ability to understand social work theoretical frameworks of practice aligned with ATSICHS values.
  • Ability to deliver client interventions that build client motivation insight and acceptance selfawareness decision making and sustainable behaviour change.
  • Ability to apply technical knowledge of relevant legislation (child protection/housing/youth justice etc).
  • Ability to proactively engage with/develop strong working relationships with external stakeholders including Child Safety Youth Justice Schools and Hospitals.
  • Ability to identify numerous internal ATSICHS programs and services to activate wraparound care for clients tradingoff and negotiating resources across the organisation and between other teams to achieve client impact.

Additional Requirements

  • Requirement to travel across the Logan and/or Brisbane region as per program location.
  • Proof of work rights in Australia.
  • Current drivers licence.
  • Working with Childrens Card (Blue Card) or be eligible to apply.
  • Valid Police Check.
