Although it serves a variety of consumers the agency focuses primarily on providing care to lowincome highneeds atrisk children youth families and adults.
Enhanced Care Management (ECM) is a benefit available to MediCal managed care Members under specific populations of focus for children/ families and adults to address social drivers of health. ECM is an intensive care coordination process to coordinate care and services among the physical behavioral dental developmental and social services delivery system. The role of the ECM Outreach Specialist is to outreach and engage potential members to provide information on program offerings eligibility and enrollment. ECM services are provided to improve the health and wellbeing of Members and increase skills and overall functioning levels of Members developed through an individualized Membercentered process upon enrollment into the program.
Starting Pay Range:
$21 $25 per hour (starting pay will be based on previous work experience and educational background.
*This is a communitybased position out of Pasadena CA that requires consistent Countywide Travel for Outreach Activities
