Job Description

GENERAL OVERVIEW:

Under the direction of the Operations Supervisor and as a member of the Unit Based Patient Care Team the Unit Coordinator will be responsible for the following duties including but not limited to answering unit phones scheduling patient tests and appointments maintaining all medical records coordinating discharges and preparing diet sheets. The Unit Coordinator is responsible for greeting and receiving visitors and responding to needs of patients and family members and taking a proactive role in the efficient operation of the unit.


PRINCIPLE DUTIES AND RESPONSIBILITIES:

SERVICE:

1. As a member of the Patient Care Team the Unit Coordinator is responsible for ensuring that the patient and family members are attended to in a caring and attentive manner. The Unit Coordinator is responsible for responding to the needs of the patient/family members in a timely manner and maintains a professional and friendly demeanor at all times. The Unit Coordinator will work cohesively as a member of the Patient Care Team and will take a proactive role in ensuring the smooth and efficient operation of the unit.

2. Responsible for greeting all patients and families and providing a complete unit orientation and responding to initial questions from family member and patients.

3. Responds to the individual needs of patients/families.

4. Maintains hospital service standards with particular attention to personal/behavioral staff teamwork and patientstaff interaction guidelines.


PRINCIPLE DUTIES AND RESPONSIBILITIES:

1. Responsible for answering coordinating and managing all phone calls to the unit. Places pages and phone calls upon request of clinical staff.

2. Coordinates all paperwork coming onto unit including all fax referrals.

3. Prepares and maintains all patient medical records in a neat and orderly manner; properly label each page and file laboratory and other information in individual record.

4. Responsible for scheduling all patient diagnostic tests appointments and consults upon direction of physician or RN. Arranges patient escort and transport services.

5. Coordinates all logistics of patient discharge including checking on family notification transportation equipment prescription and followup appointments.

6. Perform clerical duties to admit transfer and discharge patients. Make certain that patient record/chart information plate personal belongings and requisitions accompany the patient on transfers.

7. Prepares all diet order sheets and submits for approval from physician or RN.

8. Act as a liaison and assist in problem solving with ancillary services and all other departments which provide services or have an effect on unit activities.

9. Provides back up coverage for patient transport including escorting discharge patients/family.

10. On a daily basis monitors and ensures that Advance Care Directives are up to date and in the patient chart.

11. Performs all other duties as assigned.

QUALIFICATIONS:

1. Associates Degree preferred. High School Diploma or equivalent required. However a student with appropriate judgement and other qualifications (ability to complete job functions etc) can be considered for this position with appropriate review and approval.

2. 12 years of experience in medical customer service setting preferred but not required.

3. Knowledge of Medical Terminology

4. Computer knowledge in windowsbased environment strongly preferred.

5. Demonstrated customer service skills


SKILLS AND ABILITIES:

1. Excellent interpersonal skills to interact effectively with patients visitors and hospital personnel.

2. The ability to work as a team member promoting a caring and collegial attitude toward fellow employees.

3. The ability to work under stressful conditions adapting to the changing pace on the pod frequent interruptions emergencies and critical ill patients.

4. Ability to work independently with well developed organizational skills in order to prioritize tasks.

5. Demonstrated verbal and written communication skills to effectively interact with staff through voice mail email etc.

6. Demonstration of flexibility and willingness to adapt to diverse role as a member of the pod based patient care team.

7. Must be able to demonstrate the knowledge and skills necessary to provide indirect care appropriate to the age of the patients served on his/her assigned unit. The age of patients on this unit range from Adolescent to Geriatric.

8. Ability to sit and/or stand for an extended period of time.

9. Ability to assist lifting and carrying linens up to 20 lbs.

10. Ability to assist with the transportation of patients on and off the pod.


WORKING CONDITIONS:

Works in the patient care environment where there is some exposure to communicable disease and in some areas sealed sources of radiation and radioisotopes.

Willing to orient to Ante as well

Saturday and Sunday coverage
