To provide comprehensive health care for hospitalized adult medicine patients admitted to the Hospitalist Service. The nurse practitioner (NP) assists individuals and families with their health needs by obtaining health and medical histories performing physical examinations and diagnosing health problems. The NP cares for patients with acute and chronic diseases by managing therapeutic regimens prescribing medications and performing other studies and activities that are deemed appropriate. New and followup patients will be seen and evaluated by the NP and physician input will be obtained as necessary as described in the nurse practitioner practice guidelines.
The NP will assume case management responsibilities and will facilitate the referral process for patients requiring multidisciplinary evaluations. The NP will work in collaboration with the patients attending physician under the direction of the Hospitalist or designee at all times. Patients evaluated by the NP include those patients admitted to the medical service under the Hospitalist Program and those patients on various other services for whom medical consults have been requested for the Hospitalist Program by the attending physician. The patient population includes patients 17 years of age and older. Patients evaluated by the NP in the inpatient setting and emergency department include those with acute or chronic medical behavioral and psychosocial problems.
