Job Description

ReEntry Case Manager

GENERAL STATEMENT OF DUTIES AND RESPONSIBILITIES:

1. Manages and assists inmates with services and referrals to education and program service opportunities available in the Department and the surrounding communities;
2. Provides guidance on personal classification recovery and general legal matters. Identifies and resolves potential problems not immediately apparent to the inmate (e.g. discharge date calculation and canteen classification parole or discharge issues); and
3.

Must be able to travel to locations when requested and/or needed ie; House of Corrections Nashua Street Jail Portland Street.

Reports to Director of ReEntry

Works under the general supervision of the Director of ReEntry and engages and interacts with Mens Social Services staff the Substance Abuse Program staff the Womens Program staff or one of various Social Services caseworkers.

DUTIES AND RESPONSIBILITIES:

  1. Manages program partners and assists with case load management and facilitation of programs.
  2. Works to create new partner programs meets with external partners and assist with partner evaluations.
  3. Deliver and coordinate reentry efforts for birth certificates foods stamps Mass Health Mass DOR child support services toiletry bags etc.
  4. Coordinate workshops and services with community partners
  5. Coordinate transportation for returning citizens to home parole office or a residential treatment program
  6. Residential placements via sober houses/halfway houses parole and probation advocacy
  7. Coordinate monthly Discharge Panel for individuals 90 days to release with our community partners
  8. Provide workforce development training and job placement
  9. Coordinate job fairs and referrals to MassHire Downtown and MassHire Boston
  10. Refers inmates to program and support services including but not limited to recovery meetings religious services life skills and educational programs medical and legal services.
  11. Assists inmates in their postrelease plans relative to housing employment education recovery and medical services available in the community.
  12. Prepares case notes and maintains accurate and up to date records on to his/her caseload.
  13. May be tasked to work with Probation officers in District Court (Drug courts) to improve the continuum of care of inmates upon release. If so will meet with court personnel probation officers a recovery team and the presiding judges and prepare recovery packets and all releases of medical information. Will also meet with the court regularly on reentry issues.
  14. Performs such additional duties as the Sheriff or designee may assign from time to time.

QUALIFICATIONS:

1. Considerable knowledge of community resources and programs available to inmates.
2. Sensitivity to individual emotional and behavioral patterns and the impact of cultural racial educational and economic factors in society.
3. Working knowledge of institutional educational classification and therapeutic processes required.
4. Considerable ability to conduct large and small recovery meetings; to listen attentively and work effectively with inmates; to maintain a harmonious relationship with staff community agencies and clients; to maintain case records according to specific guidelines; to recognize specific educational and recovery needs; to maintain caseloads as assigned and to keep detailed records logs and files.
5. Working knowledge of the bail process court process and sentencing procedures preferred.
6. Bachelors Degree in related major preferred. Collegelevel courses in Criminal Justice Psychology Sociology or at least one 1 year of fulltime or equivalent parttime experience as a correction/jail officer or caseworker providing services to prisoners or exoffenders required.
8. Ability to communicate effectively both in oral and written form.
9. Thorough knowledge of Department policies and procedures is preferred.
10. Bilingual (English/Spanish) preferred (where applicable).







ESSENTIAL FUNCTIONS

  1. Regular punctual and predictable attendance at assigned workplace and at scheduled meetings;

    3. Compliance with security procedures;
    4. Obeying of moderately complex oral and written orders;
    5. Communicating effectively both orally and in writing;
    6. Able to independently initiate a project execute the necessary steps and complete in a timely and effective manner;
    7. Deal calmly and effectively with stressful situations including emergencies; and
    8. Exercise good judgment.



MINIMUM PHYSICAL REQUIREMENTS FOR ESSENTIAL FUNCTIONS

1. sit or stand periodically for 8 hours; lift up to 5 pounds; carry up to 5 pounds; climb stairs occasionally; walk short distances; and write and type.
