Job Description
Colavria Therapy Services provides innovative and quality rehabilitation to our 5starrated communities: The Rehabilitation Center at Sandalwood and Westlake Care Community.
We are proudly managed by Colavria Hospitality. A local familyowned company specializing in assisted living rehabilitation and longterm care in the greater Denver area.
This position is located at The Rehabilitation Center at Sandalwood in Wheat Ridge. Some travel may be required.
What youll be doing everyday:
Your primary role is to work collaboratively with your team to provide direct rehabilitation care in speech therapy to residents upon physician referral as well as to adequately plan for resident needs.
- Meet the residents goals and needs and provides quality care by assessing and interpreting evaluations and test results; determining speech therapy treatment plans in consultation with physicians or by prescription
- Administer speech therapy treatments
- Complete discharge planning by consulting with physicians nurses social workers and other health care workers; contributing to resident care conferences
- Assure continuation of therapeutic plan following discharge by instructing residents families and caregivers in home care programs; recommending and/or providing assistive equipment; recommending home health or followup programs
Who we are looking for:
You will be able to demonstrate composure customer focus patience and the ability to gain trust and establish rapport with residents and your team.
- A strong desire and passion to work in an environment with people that are unwell disabled or living with dementia. Prior experience with mental health and psych population is preferred.
- Must be a graduate of an accredited program in speech therapy (minimum MSSLP)
- Must be ASHA certified (CCCs) or must be eligible for 9month Clinical Fellowship Year (CFY) in order to earn ASHA certification (CCCs) and/or meet ASHA credentialing criteria
- One years experience in longterm care or related setting preferred

Pay range: $45:00$55:00/hour
