Mercy Health Lorain in partnership with Lorain County Public Health and supported through funding from the Health Resources and Services Administration (HRSA) is launching the Family Nurturing Center to improve maternal and infant health outcomes across Lorain County. This innovative center will provide a comprehensive wraparound care model for mothers babies and familiesfrom prenatal care through the babys first birthday.
The Family Nurturing Center Program Coordinator is responsible for the oversight development and sustainability of this program. This includes ensuring that the program is fully operational compliant with all requirements and aligned with both Mercy Health policies and the communitys evolving needs. The Program Coordinator will also lead billing efforts oversee staff training and work closely with internal teams (including IT Finance and Compliance) to build an EPIC infrastructure that supports longterm sustainability of the program.
Essential Job Functions :
Develop implement and maintain operational workflows for all program services (prenatal education postpartum care breastfeeding support care coordination community referrals).
Collaborate with IT to build and optimize EPIC infrastructure that supports seamless documentation care coordination and billing.
Establish and coordinate billing processes ensuring timely and accurate claims submission and compliance with insurance guidelines.
Develop and monitor key performance indicators (KPIs) tied to maternal and infant health outcomes financial performance and program efficiency.
Lead training programs to ensure all team members (CNPs navigators CHWs social workers etc. are proficient in processes documentation and billing requirements.
Serve as the primary point of contact with internal and external partners including funders health plans public health agencies and communitybased organizations.
Analyze program data to identify trends evaluate impact and recommend process improvements.
Ensure adherence to grant requirements including timely reporting and data submission.
Coordinate multiple projects simultaneously (system build quality improvement training development external reporting).
Serve as a coach and mentor to Family Nurturing Center staff.
Facilitate multidisciplinary team meetings ensuring strong communication across all areas of the program.
Ensure ongoing compliance with internal Mercy Health policies state and federal regulations and grant requirements.
Engage in ongoing education and training to remain current with best practices in maternal and infant health care.
Other duties as assigned to support the sustainability and success of the Family Nurturing Center.

Education Qualifications
Required Minimum Education:
4 year/ Bachelors Degree
Healthcare Administration Public Health Nursing or related field
Preferred Education:
Graduate Degree (Masters)
Public Health Healthcare Administration or Business Administration
Licensing/ Certification
Licensure/Certification Preferred:
Project Management Certification (PMP)
EPIC Certification or Super User Training
Minimum Qualifications:
Minimum of 35 years of program coordination experience in healthcare maternal/child health or community health. Experience with EPIC or similar EHR implementation and optimization.
Other Knowledge Skills and Abilities Required:
Strong leadership project coordination and organizational skills. Experience with EPIC system design/optimization. Knowledge of community health social determinants of health and maternal/child health best practices. Ability to analyze program data and make recommendations for improvement. Cultural competency and ability to work effectively with diverse populations. Advanced proficiency in Microsoft Office (Excel Word PowerPoint Teams).
