Job Description

BronxWorks helps individuals and families improve their economic and social wellbeing. From toddlers to seniors we feed shelter teach and support our neighbors to build a stronger community. BronxWorks has provided homeless outreach in the Bronx via contracts with the NYC Department of Homeless Services since 1995 Our outreach team provides mobile case management services to a caseload of over 400 chronically street homeless adults with the ultimate goal of ending their homelessness.

BronxWorks is hiring 1 Senior Case Manager for Homeless Outreach Program

RESPONSIBILITIES

  • Support the training and supervision of new and/or junior staff members
  • Maintain familiarity with social services and resources available both onsite and in the community and facilitate the sharing of these resources and information amongst staff
  • Support scheduling and admin for twiceweekly contracted psychiatric services
  • Oversee program functions in a supervisors absence as assigned
  • Independently manage a caseload Conduct comprehensive intake assessments
  • Conduct regular field visits to engage and build rapport with chronically homeless individuals
  • Work with clients to develop goals and service plans Provide referrals and direct assistance to support clients applying and/or attending appointments for: mental health services medical care substance treatment vital documents benefits legal/immigration services and permanent housing
  • Document all interactions with or on behalf of clients; monitor and document client progress toward service plan goals
  • Coordinate client care with other service providers; attend regular case conferences with clients and other service providers as required; support program leadership with concerns and inquiries received from other service providers
  • Attend regular supervision sessions with direct supervisor lesen.
  • Drive agency vehicle to and from client locations and appointments
  • Perform additional duties as assigned

QUALIFICATIONS

  • Bachelors degree in social work or a related field of study
  • Four years of related work experience
  • Candidate must have a valid drivers license which has been held for at least 1 year; in the last 3 years candidate must have obtained no more than 3 license violation points
  • Ability to engage and build rapport with populations with high support needs
  • Strong oral and written communication skills
  • Detailoriented and resourceful with strong time management and organizational skills
  • Proficiency in Microsoft Office suite and other standard business technology
  • Bilingual/multilingual preferred but not required
  • Enrollment/intention to enroll in a related masters program preferred (MSW MPP MPH MAC etc.

Physical Requirements

  • Ability to use a computer for prolonged periods
  • Ability to occasionally lift and/or move up to 10 pounds
  • Ability to stand walk or sit for long periods of time
  • Ability to bend and retrieve objects and/or documents
  • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation
  • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather including inclement weather if required to conduct home visits and/or field work
  • TB Test required within the first 120 days of employment
