GENERAL DESCRIPTION:
his role provides essential support to the community resources program by accompanying the Liaison on facility visits and managing complex cases. Youll be part of a dedicated team ensuring that all case documentation meets the high standards.
ROLE AND RESPONSIBILITIES:
QUALIFICATIONS AND EDUCATION REQUIREMENTS:
High School diploma or GED with 1 year of related work experience and/or interest in working with nursing facility residents including older adults or persons with disabilities who are transitioning from an institutional to a community setting.
The ideal candidate will have the ability:
A valid drivers license and a legally insured and registered motor vehicle are required a copy of current license and registration will be on file in the personnel record.
ESSENTIAL FUNCTIONS:
Sufficient clarity of speech and hearing which permits communication with supervisor coworkers consumers community professionals and general public.
Ability to talk on a phone stand walk sit and travel in the community.
Ability to travel throughout the state of Massachusetts in a private vehicle under all weather conditions including snow.
Ability to express ideas concisely and clearly orally and in writing.
Sufficient vision which permits production and review of a wide variety of materials written correspondence reports and related electronic and paper communication.
Ability to lift and carry objects weighing 20 pounds.
Ability to read and analyze data.
Ability to exercise sound judgment in making decisions.

Work schedule for this role: 21 hrs. per week; Mondays Tuesdays and Wednesdays from 9am5pm.
