Job Responsibilities
Under general supervision provides a broad range of paraprofessional social services to patients/clients/consumers and their families. Conducts patient/client/consumer intakes participates in treatment team planning and performs crisis intervention. Assists in dealing with personal and social problems. May provide supportive counseling to consumers and families and/or serve as a liaison for social services. May perform case management duties.
Primary Duties & Responsibilities:- Assists clients in locating and utilizing community resources including legal medical financial assistance and other referral services.
- Completes patient/client/consumer intakes based on interviews with patients/clients/consumers their families significant others and appropriate community agencies.
- Counsel clients living in group homes and halfway houses; assists in prerelease and release planning relating to clinic services.
- Implements and organizes the delivery of specific social services within the community.
- Maintains contact with other social service agencies and health care providers involved with clients to provide information and obtain feedback on clients overall progress.
- Maintains program statistics for purposes of evaluation and research.
- Monitors patients/clients/consumers progress towards treatments goals and performs follow up at regular intervals.
- Prepares intake reports and case history records.
- Provides shortterm case management and referral services to clients with emergency situations.
Minimum Qualifications
High School diploma/GED and two 2 years of job related experience in a social servicesrelated position; or One 1 year of experience at the lower level Social Svcs Tech 1 (SST010 or equivalent position.
Peer Advocate should be members of the HIV/AIDS community (PLWHA People living with HIV/AIDS)Preferred Qualifications:
- Basic computer skills (Microsoft Word & Excel experience)
- Access to reliable transportation
- Have appropriate skills relevant experience cultural and linguistic competency knowledge about HIV/AIDS & client confidentiality and knowledge of available health and social services related resources
- Prior participation in consumer leadership training

Georgia Department of Public Health Commissioner and leaders encourage all employees to engage in regular wellness activities and to make lifestyle choices that promote health and wellbeing.
The use of wellness breaks during the workday is authorized to support this philosophy and assist employees in meeting their wellness goals. A maximum of 30 minutes in a given workday may be used to engage in wellness activities generally in the form of two 15minute breaks or one 30minute break.
Employment Information
All approved teleworking employees must reside within the state of Georgia and depending on the responsibilities of the position must live within reasonable driving distance of the agencys primary worksite and be able to report to the office when required.
Current State employees are subject to State Personnel Board rules regarding salary.
DPH accepts educational credential recognized by the Council for Higher Education Accreditation (CHEA) and/or the US Department of Education (DOE) will be considered. DPH will contact educational institutions to verify degree diploma licensure etc.
The candidate selected for this position may be subject to preemployment drug screening and a criminal background check.
