The Amyotrophic Lateral Sclerosis multidisciplinary clinic is a busy ambulatory clinic in the department of neurology. There are currently 10 providers 4 nurses 2 physical therapists and 2 speech pathologists. Patient volume is high with roughly 500 patients seen a year. The patient population requires a multidisciplinary team approach. The ALS resource Specialist is involved in designing a program that will meet the needs of our patients both in clinical care and in community resources. The position requires a high degree of flexibility independence and willingness to participate in multiple activities and provide support to patients by working with the team. The position must have strong communication skills and the ability to converse comfortably with patients and their caregivers practice staff and program staff and administration. The ALS Resource Specialist would work with various community groups including but not limited to the Muscular Dystrophy Association to ensure enrolled patients are receiving the services they need to achieve an optimum quality of life.
PRINCIPAL DUTIES AND RESPONSIBILITIES: Research resources and programs available for patients and their caregivers including but not limited to:Disabled housing assistance and programs including home modifications.Insurance resources assisting in expedited Medicare processing.Health education material.Uploads and maintains resource database. Regularly updates staff on new information.Work collaboratively with the multidisciplinary team to support ALS patients. This may include but is not not limited to:Conduct follow up phone calls after clinicResearch and find DME contractors and resources for patient specific equipment.Contact community resources and act as liaison for patients.Research and find support systems for patients and caregivers experiencing grief or bereavement.Identify and refer to community services such as transportation and PCA.Coordination referrals to internal and external resources.Maintain a database on specific referrals and provide report as required.Triage services requests and forward them to other services available to the program as necessary.Patient Education and AdvocacyRefer patients to education regarding the health care delivery and reimbursement systems prescription drug programs health & wellness programs long term care insurance health care proxy government programs community agencies public & private organizations housing options and other services as appropriate.Advocate for patient to ensure safety physical financial wellbeing. Support their caregivers & quality of life goals help navigate the healthcare delivery system obtain third party reimbursement for covered goods and services etc.

LICENSES CERTIFICATIONS and/or REGISTRATIONS (if applicable): LISW preferred will accept experience without license
EDUCATION:Bachelors degree
EXPERIENCE:2 years experience as case manager and/or related field
