TheStaff Physician Assistant will work with the Interventional CardiovascularMedicine Service and particularly with the Interventional Team within theCardiovascular Division of the Department of Medicine. This individual will assist the attendingcardiologist in all areas of patient care in the Interventional CardiovascularMedicine Service.

Clinical Duties:
PreprocedurePatient Care:
TheStaff Physician Assistant will be responsible for obtaining and coordinatingthe following general items: history and physical laboratory results chartingof clinical findings admission orders nursing instructions and other requiredelements. The Staff Physician Assistantin collaboration with the Laboratory Flow Managers will coordinate admissionsand transfers to the hospital and ensure that informed consent for proceduresis obtained. The Staff PhysicianAssistant will perform preprocedure assessments in the Preadmission testingcenter Recovery room/holding area or outpatient setting as needed.
Interfacebetween Cardiac Catheterization and Intervention Laboratory and Recovery Area:
TheStaff Physician Assistant will be knowledgeable about the CardiacCatheterization and Intervention Laboratory scheduling procedure room turnoverand availability of resources. He/shewill be called upon to manage patients after their procedures and while theyawait transfer to patient floor and will assure availability of equipment forspecial procedures. The Staff PhysicianAssistant will ensure blood bank preparation and formal or standing surgicalbackup when required for patient care.The Staff Physician Assistant will assist in family liaison.
MedicalTreatment and Postprocedure Care:
TheStaff Physician Assistant will be responsible for many aspects of care underthe supervision of the attending physician including but not limited to: fluidand electrolyte management nutrition pain management transfusion therapyanticoagulation management education and postdischarge planning. Following procedures the Staff PhysicianAssistant will manage vascular access monitor and manage arrhythmias pacingdrug therapy O2 therapy and other noncardiac medications underthe supervision of the attending physician.The Staff Physician Assistant will assist in the evaluation andmanagement of clinical events such as fever altered hemodynamics CHFpulmonary complications abrupt vessel closure arrhythmias access sitecomplications and others. The StaffPhysician Assistant will assist in transfers to and from the intensive careunits.
Procedureresponsibilities:
TheStaff Physician Assistant will assist the attending physician duringdiagnostic interventional and research procedures performed in the CardiacCatheterization and Intervention Laboratory and other clinical areas.
DischargePlanning:
TheStaff Physician Assistant will perform patient education regarding dischargeincluding preventive care and risk factor reduction. He/she will write discharge ordersprescriptions and dictate discharge summaries in collaboration with theattending physician. The Staff PhysicianAssistant may act as a liaison between the attending cardiologist and familymembers and will schedule followup care as necessary including VNA ifneeded.
Researchand Education:
TheStaff Physician Assistant will be offered the opportunity to participate inaspects of research involving planning performance and analysis of data andpublications. The Staff PhysicianAssistant will complete the 100 hours of continuing medical education necessaryduring each 2year period of employment.The Staff Physician Assistant will attend internal and externaleducation meetings and read appropriate professional journals in order tomaintain and enhance skills and clinical competence. The Staff Physician Assistant will be encouragedto advance his/her medical and technical skills as appropriate to deliveroptimum patient care.
QUALIFICATIONS:
Must havesuccessfully completed a Graduate Level accredited Physician Assistant Programapproved by AMA Board of Approval and Certification of PA programs.
Must have 4 yearsof college level education and bachelors degree or equivalent. (BS/MS jointprograms are also acceptable; BS with PA certificates are acceptable forgraduates before 1995.
BoardCertification by the National Commission on Certification of PAs is required.
Cardiologyexperience preferred.
Specificlicense/certification/registration required to perform the job.

c.Maintain currentDEA and Massachusetts Controlled Substance Licenses
d. Complete 100 hours of CME according to national andstate regulations during each 2 year period of employment.
e.Maintain currentBLS/ACLS certification
f.Complete BWHProfessionalism Program within 3 months of date of hire.
g.Complete BWHProcedural Sedation Training if determined necessary.
SKILLS/ ABILITIES/ COMPETENCIES REQUIRED:
List special analytical administrativetechnical and/or financial abilities or skills required.
EachPA shall demonstrate the following professional knowledge and skills:
a. Proficient with BWH computer systems as needed toindependently perform daily tasks such as order entry EMAR lab tests andresults incident reporting BICS etc.
b. Consistently provide high quality and timelydocumentation including admission notes progress notes discharge summariesand dictations.
c.Must have knowledgeof billing practices.
PAs:
Must beenergetic quality driven and have proven ability to be a productive member ofa cohesive team of peers dedicated to providing optimum patient care.
Must exercise thehighest degree of professional judgment under the direct supervision andguidance of a licensed physician.
Must maintain aprofessional attitude towards patients and exercise such mature independentoriginality in solving problems not covered by existing guidelines withinlegal limits to meet the needs of the patients.
Must maintain aprofessional attitude towards patients at all times and an excellent andcollegial working relationship with other personnel involved in patient care.
Must demonstrateunderstanding of procedures policies and documentation required to ensurecompliance with hospital standards of care.
EachPA shall demonstrate the following clinical knowledge skills and competencies:
a. For the most common diagnoses admitted to theInterventional Cardiology Service demonstrate competency to effectivelydiagnose formulate appropriate treatment plan and efficiently implement theplan without much direction and with a greater level of independence from theattending physician.
b. Clinical skills and knowledge as well asprofessionalism of the PA candidate are assessed by Interventional Cardiology Attendingunder the direction of the Cath Lab Medical Director.
c.Candidates withtwo or more years of relevant PA experience prior to joining the service becomeeligible for a preliminary evaluation of competencies approximately 3 monthsafter joining the service. The purpose of the preliminary evaluation is toprovide feedback to the candidate on progress towards level of competencyrequired and provide guidance on additional development needs. Afterapproximately 6 months after joining the service the candidate then becomeseligible to complete the assessment and if successful advances to a PAII.
d. If a candidate does not pass the competencyassessment cath lab attending MDs will provide feedback on additionaldevelopment needs. Reassessment is possible after 6 months of focuseddevelopment.
e.Competencies areassessed by cath lab attendings either through direct observation or verbalcommunication. The attending physician(s) performing the assessment will completea competency assessment. The candidate is responsible to provide example caseswhich he or she has cared for patients with the most common diagnosis
WORKING CONDITIONS:
Work requires a considerableamount of walking and standing while wearing heavy lead.
Requires physical effort tolift and position patients and radiographic equipment.
Exposure to some radiationduring fluoroscopy spot filming and digital acquisition in labs.
Exposure to blood and wasteproducts and contagious diseases.
Constant changes in lighting overheadlighting followed by dark during flouroscopy bright lights on illuminatorsand surgical lamps dark in the reading room etc.
Stressful highvolume highacuity patient care environment.
