Certified Home Health Aide (CHHA)
Territory: Napa Area
Under the direct supervision of an RN Case Manager you will provide patient care according to the Plan of Care.
What youll do as a Certified Home Health Aide (HHA):
- Provide personal care including baths back rubs oral hygiene shampoos and changing bed linen
- Assist patients with dressing and undressing
- Provide skin care to prevent breakdown
- Assist the patient with toileting activities
- Keep patients living area clean and orderly as appropriate
- Assist in feeding the patient if necessary.
- Perform range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency).
- Assist patient in the selfadministration of medication.
- Meet safety needs of patients and using equipment safely and properly (foot stools side rails etc..
- Report on patients condition and significant changes to the assigned nurse.

To be a success as a Certified Home Health Aide
- Valid CHHA CA Certification
- Current BLS/CPR Certification
- Valid CA Drivers License
- Excel at building rapport with patients and providing the highest quality of care
- Experience home health care is highly preferred
- Experience with high acuity patients is a plus
- Experience working with an interdisciplinary team is highly desired

- Prior to scheduling an inperson interview all Candidates are required to complete a computer literacyand competencyassessment. This is to ensure you are set up for success and can navigate in our environment including electronic chartingshared understanding of company valuesand use of our web based communication application. The results of the assessment are confidential and will be used as one of multiple data points included in the hiring decision process.
$55000 $90000 a year
Estimated Wage range Certified Home Health Aide (CHHA): 55000..00 Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans comprehensive health benefits and 401k (Up to 4 match)
*You will be assigned to a primary location however you will be expected to be available to cover for vacancies leave of absences as well as supporting increased census and client care needs outside of the assigned area. 21st Century Home Health puts forth every effort to accommodate the assigned primary territory but on occasion in order to support safe patient care and meet the demands of a growing business staff may be reassigned temporarily.
