Social Worker

Responsibilities
- Build longterm relationships with residents and their families with a personcentered focus.
- Create and maintain atmosphere of warmth personal interest and positive emphasis
- Act as a liaison between hospitals rehabilitation centers and RiverWoods Healthcare to coordinate smooth admissions of RiverWoods residents into healthcare including room management.
- Provide residents with information concerning resident rights living wills power of attorney documentation advanced directives RiverWoods Lodge Handbook Longterm care Ombudsman information and other pertinent information upon a residents admission to healthcare.
- Complete all admission paperwork and consents in accordance with federal and state requirements as well as RiverWoods established policies and procedures.
- Participate in the development of a written interdisciplinary plan of care for each resident that identifies social emotional psychosocial and spiritual needs/issues of the resident the goals to be accomplished for those needs/issues and the appropriate social worker intervention.
- Ensure or provide therapeutic interventions to assist residents and their families in coping with their transition and adjustment to the healthcare facility including their social emotional psychological and spiritual needs.
Qualifications
- Minimum 4year college degree and BSW required with a preference for MSW degree
- Minimum of 5 years of experience in a longterm care setting healthcare or related field
