Reports to: Senior Director Housing Stabilization

Program: 1704 Prevention/Diversion

Department: CES Homeless Prevention

Location: The Nest II 316 W 2nd Street Los Angeles CA 90012

Setting:Not100 Onsite

Schedule:Monday Friday 8am 4:30pm

SUMMARY:

The Coordinated Entry System (CES) Homelessness Prevention program assists participants who are at risk of becoming homeless due to being in the early stages of the eviction process by assisting to retain permanent housing. This program essentially diverts participants from entering the homeless shelter system by providing housing assistance benefits and linkage to legal services to participants who are at imminent risk of homelessness in coordination with regionally located CES lead agencies.

This position In coordination with the Director of CES and Rapid Rehousing would assist in leading a team in resolving clients housing crises that would otherwise lead to a loss of housing by providing services including but not limited to short term financial assistance housingrelated support services legal assistance and client care plans that lead to housing stabilization. This position will also be responsible for building and maintaining good working relationships with landlords/management companies/property owners as well as with other homeless service providers and partnering agencies.

Essential Duties and Responsibilities:

  • Provide supervisorial support and oversight for staff related to caseloads scheduling data integrity and contract goals or program changes.
  • Oversight and quality assurance of caseloads client files and housingrelated documentation
  • Assess housing barriers of participants who are unstably housed and atrisk of homelessness to determine housing and service needs by use of assessment tools.
  • Develop housing procurement financial and selfsufficiency case management plan with participants. This will include an intake interview/assessment to determine household needs goals and eligibility.
  • Determine eligibility and appropriate level of assistance based on client situation and ongoing monitoring of program spending and budget.
  • Communicate team needs updates and maintain the tracking of reports as requested by supervisor to ensure project deliverables are being met.
  • Use progressive assistance lens when reviewing and monitoring financial requests for the program to ensure that requests are in alignment with scope of services as outlined by our funders and that each request appropriately meets the needs of program clients
  • Stay informed on the Scope of Required Services (SRS) as outlined by our funders.
  • Market promote and network with providers in service planning area 4 (metro LA) to improve program partnerships and coordination.
  • Create and maintain consistent communication channels both verbal and written between several parties (i.e. tenant landlord referral source collaborating agencies debtors and creditors and most importantly staff)
  • Serve as an ongoing liaison between property managers legal supports and participants as well as between participants and neighbors.
  • Apply knowledge of residential lease contracts to educate participants of their tenant rights and responsibilities.
  • Works in conjunction with legal service entities to facilitate the work flow between case management and legal service support.
  • Credit counseling and other services necessary to assist program participants with critical skills related to budgeting managing money accessing a free personal credit report and resolving personal credit problems.
  • Documents all services identified and provided referrals made and transportation provided.
  • Advocate for and actively assists individuals in obtaining services (e.g. benefits mental health substance abuse housing referrals financial assistance home based services training medical services mentoring and socialization).
  • Travels and makes home visits as well as work an adjusted work schedule including evenings and weekends as necessary.
  • Ensures data is being entered and tracked in an accurate timely manner. This means keeping data in HMIS charts and productivity trackers up to date.
  • Other duties as assigned

Qualifications:

  • High School Diploma or equivalent and one year fulltime experience working with the homeless preferably those living with mental illness and / or substance addictions.
  • Knowledge of homeless social service providers and Coordinated Entry System
  • Able to communicate and interact with subcontractors housing providers and community partners in a culturally competent manner
  • Able to work with and lead community groups and case managers
  • Strong background or experience with Microsoft Office

Preferred Qualifications:

  • Bilingual Spanish preferred
  • Bachelors degree in related field (Social Work Community Organizing or other Human Services)

Work Environment:

  • Combination of field and office environment
  • Regularly required to sit stand bend; occasionally lift or carry up to 35 lbs.
  • On occasion walk or drive to different local sites
  • Will necessitate working in busy and loud environments
  • Will be exposed to elements like cold heat dust noise and odor
  • May need to bend stoop twist and sit throughout the day
