Job Description
SUMMARY:
Under the direction of the Director of Health Services provide individualized nursing care to participants enrolled in the Adult Services Program. As a licensed practical nurse you will be responsible for planning organizing and developing all nursing activities.
DUTIES AND RESPONSIBILITIES:
- Responsible for the overall operation and optimal quality of care participants
- Follow a designated care plan in accordance with patients needs
- Perform assigned duties
- Including administration of medication wound care treatments and procedures
- Monitor clients conditions; reporting changes to RN and/or caregivers
- Follow up with execute and properly document orders
- Accurately document observations interventions and evaluations pertaining to client care management and services provided
- Follows all health sanitary and infection control policies and maintains established standards of practice set forth by the facilitys administration and the Nursing Policies and Procedures
REQUIRED SKILLS AND EXPECTATIONS:
- Ability to work cooperatively with all members of the staff.
- Ability to obtain and organize resources that meet the needs of elderly and disabled participants.
- Ability to train participants caregivers CNAs and staff in health related matters.
- A proven ability to work effectively and respectfully with a broad range of people from diverse cultural ethnic and socialeconomic backgrounds.
- Ability to engage and motivate program participants to increase/maintain their selfreliance and work toward resolution of the challenges they face.
- Ability to advocate on behalf of program participants and demonstrate a belief that participants have the capacity to grow and change.
- Respects the integrity of each program participant their right to privacy and confidentiality and their right to participate to their maximum capacity in determining their participation in our programs including the formation of a case or treatment service plan.
- Maintains confidentiality with respect to the participant personnel and proprietary agency information.
- Able to obtain and maintain Adult CPR and First Aid certification.
JOB REQUIREMENTS:
- Valid Louisiana drivers license.
- LPN in the State of Louisiana.
EDUCATIONAL & EXPERIENCE REQUIREMENTS:
LPN certification required. Three to five years experience working in an accredited licensed medical facility
