Job Description:
We are seeking a compassionate and experienced Pediatric Nurse Practitioner (NP) to join our dynamic team at Foundation Medical Partners / Southern NH Health. In this role you will play a vital role in providing comprehensive care to a diverse patient population in Nashua NH.
Responsibilities:
- Conduct Outpatient comprehensive assessments.
- Manage desktop for refills and triage questions by the nursing team.
- 36 patient contact/4 administrative hours per week no nights or weekends
- Collaborate with physiciansnursesand other healthcare professionals to ensure optimal patient care.
- Document patient encounters and progress notes accurately and thoroughly.
- Stay up to date on the latest advancements in cardiac care through continuing education and professional development.
Qualifications:
- Masters degree in nursing
- 2 years of Family Medicine experience
- Strong clinical skillsincluding assessmentdiagnosisand treatment planning.
- Excellent communicationcollaborationand interpersonal skills.
- Proficiency with EPIC EMR preferred
- Ability to work independently and as part of a team.
