Job Summary
The University of Kentucky Department of Family & Community Medicine is seeking a qualified family physician to join our Lexingtonbased clinic located at Fountain Court to practice primary care nonsurgical obstetrics. Family and Community Medicine has several clinics in the state including additional locations in Lexington as well as clinics located in Georgetown Hazard and Hindman Kentucky. We serve as leaders at UK HealthCares $4 billion health system. We have aligned our clinical approach to enhance valuebased quality measures through previsit planning comprehensive nurse assessment and a UK HealthCare Population Health team involving social workers community health workers and RN care managers. We are a national leader in transitional care management and its impact on unnecessary hospital readmissions. Our departments clinical volume includes over 100000 annual visits and over 2500 hospital discharges across our current clinics and hospital services.
