Description of the role:
The Nurse RN for home care with oasis trained at University Home Care is responsible for providing skilled nursing care to patients in their homes. The nurse will collaborate with the interdisciplinary team to develop and implement individualized care plans ensuring the physical emotional and spiritual wellbeing of the patients.
Responsibilities:
- Conduct comprehensive assessments of patients medical conditions and needs
- Administer medications and treatments as prescribed by physicians
- Monitor and evaluate patient progress adjusting care plans accordingly
- Educate and support patients and their families regarding disease management and selfcare techniques
- Coordinate and communicate with other healthcare professionals to ensure continuity of care
Requirements:
- Valid Registered Nurse (RN) license in the state of Michigan
- Extensive experience in home care nursing
- OASIS trained and proficient in comprehensive assessments
- Excellent clinical skills and ability to provide compassionate care
- Strong communication and interpersonal skills
