Title: Behavioral Health Care Coordinator
Location: 703 NE Hancock St. Portland OR 9721
Schedule: This is an Inperson position Monday through Friday 8:00am 4:30pm
Wage Range: $34.61 $40.87 NonExempt Hourly

Position Overview:
The Spirit of Healing Behavioral Health Care Coordinator works independently and collaboratively with a team of health professionals to provide members with opportunities to improve selfmanagement abilities health outcomes and satisfaction with health care in a culturally specific manner. This position works collaboratively with American Indian or Alaskan Native members who may have complex medical/psychosocial/ addiction issues and their families/caregivers as well as with members of multidisciplinary teams (MDT) to provide care coordination and mental health assessments as needed.
What you will do:
- Actively collaborate and coordinate Care Coordination for Spirit of Healing Members/NARA NW patients utilizing a team approach
- Administer mental health assessments and treatment plans as needed to streamline member into services
- Respond to requests for outreach at local hospitals and within the community
- Help coordinate patient transportation and accompaniment as needed.
- Utilization of trauma informed practices
- Responsible for scheduling patients discharging the hospital or inpatient settings
- Work independently to perform timely multidisciplinary case management assessment of members in multiple settings (e.g. home hospital clinic etc. by evaluating all relevant information to determine needs in health care social supports and access to behavioral health services
- Provide timely updates regarding the plan of care that includes selfmanagement as well as member specific goals.
- Makes referrals to community programs/services as appropriate.
- Utilize the Electronic Medical Records system for documentation. Make independent decisions regarding client care and care management. Complete appropriate documentation in a timely manner.
- Barrier Review and Reduction. Meet with members to identify and reduce or eliminate barriers to care (e.g. social cultural linguistic education) and set effective goals with members. Utilize a culturally sensitive personcentered approach in working with clients by respecting their selfdetermination and autonomy and exploring their ambivalence by utilizing Motivational
- Interviewing principles. Ensure that clients get timely access to appropriate services by promoting and modeling advocacy skills.
- Provider and Member Integration. Collaborate with medical and auxiliary providers in the community to develop health care plans/goals based on identified needs and employ creative strategies to address psychosocial issues.
- Facilitate communication and positive relationship building between clients and medical providers; attend medical appointments as needed.
- Work Management and Evaluation. Work autonomously in identifying tasks developing plans following through on needs and evaluating outcomes. Report on progress and concerns as appropriate.
- Follows departmental organizational and clinic policies and procedures.
- Up to 80 of the workload will be performed in the community
- Manages an efficient and effective case load.
- Effective at establishing and maintaining key community partnerships.
- Reports to work as scheduled and follow attendance policies; maintain agreed upon work schedule.
- Meets deadlines for completion of daily weekly and monthend reporting.
- Demonstrates cooperation and teamwork.
- Occasional evening and weekend hours required
Qualifications
We would like to hear from people that have:
- Masters Degree in Counseling Psychology Social Work Marriage and Family Counseling or related clinical field
- Minimum 2 years experience in behavioral health care management and care coordination and/or social services.
- Active Oregon license (LCSW or LPC) or ability to obtain by start date
- Valid drivers license acceptable driving record and automobile liability coverage or access to an insured vehicle; Ability to operate a motorized vehicle
- Demonstrated skill in word processing and computer skills and ability to learn new technological means of communication and various data base
- Ability to maintain professional boundaries with clients the community and other staff members.
- Must have two years of experience using an electronic health record
- Ability to pass a preemployment or for cause drug tests.
- Ability to pass criminal background and DMV checks.
- Must have valid drivers license and active auto insurance coverage

If in recovery must have been sober/clean for a length of time sufficient to demonstrate that will be able to model sobriety.
