Plan organize develop and direct the overall operation of the Social Services Department in accordance with current federal state and local standards guidelines and regulations and established policies and procedures of Winner Regional Health that the medically related emotional and social needs of the residents are met and maintained on an individual basis and resident rights are met. Complete resident interviews and assessments for MDS and related cycle as assigned (CAAs and care plans). Schedule attend and participate in care plan conferences. Coordinator mental health support services. Oversight of plan for prevention of abuse neglect and exploitation. Coordinate grievance and concern follow up and tracking. Assist with behavior management interventions. Attend and participate in LTC Medicare meetings and notify residents/responsible parties of changes in the status including issuing Medicare notices related to coverage. Work with QAPI for overall facility improvement. Document services provided in resident medical records. Other duties as assigned.
Education/Experience:
Bachelors Degree from an accredited school of social work or a Bachelor degree in human services including but not limited to sociology special education psychology rehabilitation and counseling.
Required Credentials (Licensure Certification or Registration):
Must be a licensed Social Worker in the state of South Dakota
Employment Variables:
Normal working hours are eighthour shifts Monday through Friday. May need to occasionally work nights weekends and holidays depending on workload. Initial Tuberculosis (TB) test and drug screening is required by Winner Regional Health. Rubella titer will also be drawn upon hire and immunization is required if no past exposure or indication of immunization.
Required to wear name tag provided by WRH and to follow the dress code of WRH.
Job Knowledge and Skills:
Ability to read write speak and understand the English language. Excellent oral and written communication skills. Ability to deal with people in a professional manner. One year supervised social work experience in a healthcare setting working directly with patients/residents. Supervisory experience.

Roles and Responsibilities of Social Services
The role and responsibility of social services is to be an advocate for each resident. To insure collaboration with other members of the interdisciplinary team to verify appointments and consultations are conducted. To provide adequate documentation of resident behaviors and interventions trailed prior to consultations. To ensure adequate communication and documentation regarding the use of antipsychotic medications and provide ongoing monitoring.
Resident Focus Ensures that Winner
Regional LTC residents are maintaining the highest quality of life possible and is vocal of resident rights. Works closely with external entities (Southern Plains providers) to ensure that residents are receiving the proper holistic care.
Financial
Focus Makes sure that residents finances are in line by working with families and government entities to ensure resident bills are paid and resident personal finances are tracked appropriately. Keeps facility beds filled as much as possible to ensure financial longevity of Winner Regional LTC.
Completes resident assessments and reviews
Identifies resident needs and plans appropriate interventions. Provides options for identified needs with area resources and services. Completes reviews and documents assessments/admission data on residents and families as needed. Meets expectations for working with all age groups.
Manages and implements measures to integrate care
Assists the team in developing plan of care with resident/family. Plans organizes and coordinates with the interdisciplinary team to provide quality services and cost effective resident outcomes. Educates resident of resources and services and implements residents choices. Collaborates with the interdisciplinary team to meet the residents and familys psychosocial needs. Demonstrates timely concise documentation related to interventions and continued care planning.
Utilizes interactive communication through care planning process
Encourages open communication between the resident and family and between the residents family and staff. Consults with agencies as needed to facilitate meeting the resident family needs by communicating residents desire and appropriate data. Coordinates timely resident/family conferences with the appropriate team members as needed. Actively participates and follows through with the interdisciplinary team.
Implements appropriate skills to meet the residents psychosocial needs
Provides emotional support to resident family and caregiver dealing with pressures/problems related to illness and treatment. Provides options for identified needs with our resources and services. Promotes mental health of residents and refers residents/families to appropriate therapy services. Helps residents maintain their sense of competence identity and autonomy. Acts as an advocate for the resident/family. Maintains resident rights and dignity.
Meets expectations for participation in functions within the department
Participates in program planning policy formation and evaluation. Serves as a role model to employees. Communicates concerns in appropriate forums and participates in decision making process for social service practice. Provides inservices as needed. Participates in peer review process. Shares research articles and other resource information with interdisciplinary team as appropriate.
Admission/Discharge Coordinator
Coordinates admissions with other departments for a smooth transition to long term care. Conducts interviews of prospective resident/family. Discharge planning in coordination with the interdisciplinary team/resident/family.

Ensures task assignments are understood and appropriate. Adapts leadership style to situations and people. Provides feedback effectively. Completes followup on action plans as necessary. Deals with behavior/performance requiring corrective/disciplinary action.

Must be active in strategic planning within the department and facility to stay up to speed with current trends in healthcare.
