Job Description

  • Graduate of an accredited schoolof practical nursing require.
  • Minimum of 1 year relevant work experience required.
  • Valid state license to practicepractical nursing required.

The LPN works as part of the patientcentered healthcare team to managepatient population anticipate patient needs assist to identify and ensurerelevant tests and patient information is timely available for physicianswhile providing realtime care and customer service to our patients in theoffice telephonically and through Patient Gateway.

Principal Duties andResponsibilities:

Works closelywith the physician/provider to appropriately schedule patients to maximizeefficiency and timely provide patient care and patient services.

Responsible forpatient and work flow management on a daytoday basis to prioritize and ensurethat the provider and other healthcare team members handle messagesprescription refills review of labs/results and forms are completed timely toeliminate batching of work.

With oversightand direction from the physician/provider coordinates previsit preparationsfor patient visits to include:

oPreparing summaryreports and assisting in the evaluation of patient needs ensuring relevantlabs and tests are scheduled and completed prior to office visits as requiredand anticipate special needs for effective communication to service vulnerablepopulations;

oEnsuring allrequired documentation is in the electronic medical record prior to the officevisit;

oIdentifying highrisk managed care or patients on population management registry to ensureappropriate protocols are followed;

oCommunicate withhealthcare team as appropriate to ensure patient needs are met prior to thevisit. Participates in the healthcareteam huddle.

oCleaningpreparing and ensuring supplies are appropriately stocked and ready for use inthe exam room and clinical areas. Cleansand sanitizes as needed.


Responds to and triages incoming telephone calls. Instructspatient and family regarding medications and treatment instructions.
