REQUIRED SKILLS:

PREFERRED SKILLS:

EXPERIENCE:
- Minimum of three years experience in a health care delivery system required with specific credentials in a health care discipline strongly preferred.
- Experience in patient safety or quality improvement preferred.
EDUCATION:
- Bachelors Degree required.
- Training in performance improvement and project management strongly preferred
LICENSURE CERTIFICATIONS or REGISTRATIONS:
Reporting to the Director of Quality Performance and Patient Safety the patient safety coordinator manages the daytoday patient safety activities for the organization. The coordinator provides support and facilitation to frontline managers and project leads in implementing patient safety initiatives. Participates in teaching and training in patient safety and quality improvement for NSMC staff at all levels. Duties include cause and process mapping chart abstraction literature review and collection of best practices. Duties require strong communication and organizational skills and the ability to work independently with minimal supervision.
JOB SPECIFIC RESPONSIBILITIES:
- Works with the Quality and Patient Safety staff and leaders as well as administrative and medical staff leaders to achieve organizational goals;
- Performs as the content expert on patient safety improvement methodologies including Root Cause Analysis and Failure Modes Effects Analysis (FMEA) and imparts knowledge of patient safety improvement process to team leaders and the team. Assists the team leader in identifying barriers to implementing improvement plans and developing strategies to overcome these barriers. Assists team leader in developing outcome measurement strategies and communication plans;
- Develops the analytic and measurement strategy for all projects undertaken including development of key metrics identification of data sources and collection of baseline data;
- Provides oversight of the hospitals safety reporting system including but not limited to streamlining of processes identification of trends and the development of meaningful reports. From these trend reports initiates patient safety related strategies to ensure the delivery of high quality safe care;
- Coordinates the Patient Safety culture surveys through collaboration with hospital staff and physicians. Ensures timely completion review of results and development of action plans. Shares reports with appropriate leadership;
- Provides ongoing information and education as necessary to medical staff nursing and ancillary staff regarding patient safety the concepts of a Just Culture including nonpunitive reporting and error prevention;
- Coordinates improvement activities related to the Joint Commission National Patient Safety Goals in collaboration with the Manager of Standards Compliance and the Chief Compliance Officer;
- Facilitates problem solving and clarification across the departments. Coordinates meetings as needed. Takes the leadership and facilitator roles as needed;
- Identifies quality and safety variances that can be abstracted. Collaborates with other quality improvement initiatives as assigned;
- Expresses recommendations and concerns in an effective professional and courteous manner;
- Identifies pertinent clinical outcome issues; analyzes documents and reports appropriately in accordance with hospital and departmental policies and procedures;
- Manages ad hoc patient safety related task forces and participates in Partners Healthcare Systems level committees;
- Assists in recruitment and orientation of new staff. Represents the department in various forums used for orientation; and
- All other duties as assigned.
