  • Masters degree in Social Work required.
  • LCSW required
  • Minimum 2 years of experience with parenting child developmentsocial work or human service work
  • Fluency in Spanish required.
  • Excellent oral and written communication skills.

Working alongwith and in support of MGH Chelsea pediatricians the Pediatric Healthy Steps SocialWorker provides support for families with children age 05 years. The Social Worker provides care giver supportto facilitate the development of attachment self regulation skills and familyresiliency. In addition the SW willprovide child development guidance to highrisk parents and other primary caregivers at well child visits with the goal of facilitating childrens healthygrowth and development.

The SocialWorker works primarily in Pediatrics and collaborates with the MGH Chelsea CommunityHealth Improvement Unit to deliver a new model of care integrating the HealthySteps and Healthy Families strategies for families that have been identified ashaving significant risk factors. Thework would be carried out through home visits office visits and throughaccompanying families to key medical and specialty visits. A background in child development socialwork early childhood education or early intervention is an importantrequirement for this position.

  • Conducts office visits either jointly with the physician/pediatric nurse practitioner or alone.
  • Conducts home visits and accompanies patients to key medical specialty and community agency appointments.
  • Works closely with the pediatric providers around care coordination goal setting counseling coaching and education about key aspect of a childs development.
  • Closely tracks progress made in ongoing checkups that look at both child development and family factors
  • Provides anticipatory guidance to parents and other primary care givers around common concerns such as early learning fussiness sleep feeding discipline toilet training etc.
  • Provides referrals and tracks followup as appropriate to help families make connections to key resources within the community
  • Facilitates parents/caregivers education groups
  • Works closely with the Healthy Beginnings team including the Home Visitors Healthy Steps Infant Parent Specialists and the MGH Chelsea Pediatric staff as a resource for developmental and parenting concerns
