Job Description

Summary: Compounding Coordinator function as an integral member of the patient care team responsible for the preparation/dispensation of medications and all other pharmaceutical services at Spaulding Rehabilitation Hospital. Work under the supervision of a staff pharmacist and collaborates with physicians nurses and other pharmacy personnel to deliver safe and optimal drug therapy maximize positive patient outcomes. Provides care/dispenses medications in accordance with the laws and regulations governing pharmacists and the practice of pharmacy outlined by the appropriate state and federal agencies. Key Responsibilities: Prepares compounds and dispenses medications to patients according to the laws and regulations of the Board of Registration in Pharmacy the Department of Public Health the Food and Drug Administration and other regulatory agencies. These practices are applied to the following situations: Preadmission referrals Admissions Processing/filling of ambulatory prescriptions Drug deceleration programs Investigational Drug Therapies/Protocols New product formulation and compounding of all noncommercially available Administration/oversight of IV additives dialysis and TPN programs Dispensation of narcotics and controlled substances/inventories Providing pharmacy support and delivering medications in emergency and/or code situations Maintains and monitors all documentation related to: Drug preparation and delivery Order entry Data collection Investigational medicine protocols Equipment management Adverse events and errors Drug inventory (controlled and noncontrolled substances) Drug use evaluation data collection and other drug therapy monitoring initiatives Acts as a consultant and a collaborative interdisciplinary team member with physicians nurses and other supportive services in the evaluation of current and recommended drug/treatment therapies for safety efficacy and appropriateness of care. This includes but is not limited to: Noting drug allergies Reviewing for potential and actual drug interactions adverse events incompatibilities length of therapy Communicates to appropriate team members/oversight committees the need for preventative or corrective actions to ensure safe administration of drug related therapies Investigates and acts on ambiguous illegible or incomplete medication orders Monitors and collects data related to research and investigational drug protocols Updates and maintains patient medication profiles to contain appropriate information/clinical data Reports adverse events to pharmacy leadership and appropriate committees Recommends alternate therapies or dosing adjustments while monitoring changes in patient conditions Provides teaching and drugrelated information to caregivers in order to facilitate understanding and to increase the likelihood of efficient care/safe outcomes Consults with referring hospital on new admissions to ensure safe and appropriate continuation of care Consults with area retail pharmacists to make predischarge arrangements for patients to obtain difficult to get medications when necessary and to discuss unique SRH formulations of medications Functions as educator and continuous learner role: Provides drug therapyrelated educational literature and standard forms (i.e. dosage forms drug availability forms) to interdisciplinary team members to enhance learning and timely delivery of therapies Develops distributes and updates compatibility charts guides to parenteral dilutions and other assistive tools to caregivers Keeps abreast of the information and changes in departmental policies Attends and participates in continuing education programs and conferences to keep current with developments in the profession and to maintain licensure Provides discharge medication and drug history counseling when appropriate Maintains pharmaceutical care expertise through membership and participation in professional organizations Participates in equipment and drug inventory management in the pharmacy and in the patient care areas by: Assuring proper storage and adequate stock of materials solutions additives devices etc. for the timely and safe delivery of medications Works in the sterile products area to ensure the accuracy sterility and stability of all compounded products Performs responsible inspections of nursing and patient care medication storage areas and of equipment maintenance logs as assigned Obtains needed and medications from outside sources on a perdiem basis Checks and fills emergency and crash carts brought to the pharmacy Prepacks and inventories all nonunit dose narcotics Destroys and discards all nonunit dose medications Reviews and stocks night cabinet All other duties as assigned. Supervisory/Managerial Responsibilities (include number and type of FTEs) Fiscal Responsibilities: (include size of annual budget) Qualifications and Experience:Must be a certified pharmacy technician.Knowledge of USP Pharmacopeia. Especially USP>795 and USP>797Education/Degree Required: High School Diploma Preferred: College degree. Experience: Required: Skills in compounding sterile and nonSterile Products. Preferred: Knowledge of pharmacy automation (Omnicell) and pharmacy system Meditech. Licensure Required: Must be a nationally certified with PTCB (Pharmacy Technician Certification Board) Preferred: Skills(Specific learned activity gained through training (e.g. computer skills keyboarding presentation CPR ACLS etc. Required: Microsoft office suites Preferred: Presentation/teaching skills.
