Job Description

SERVICE AREA: Housing & Justice Related Services (HJRS)

PROGRAM: Philadelphia Treatment Court (PTC)

JOB OVERVIEW:

The fulltime Treatment Court Case Manager will coordinate supportive services for a caseload of 4050 individuals with current forensic affiliations who abuse illicit psychoactive substances.

RESPONSIBILITIES:

  • Supervise each participant once per month or more if clinically necessary at the Treatment Court Case Management office to assist participants in meeting their recovery needs.
  • Serve as a liaison between the treatment facility and legal team (including the Treatment Court Judge Assistant District Attorney and Assistant Public Defender). Inform all parties of participants treatment progress including arrests or any other problematic behavior that might jeopardize their status with Treatment Court.
  • Prepare summaries about specific participants progress and present summaries at the bar of the court on selected Wednesdays.
  • Ensure the delivery of comprehensive services to 4050 assigned participants in the areas of housing education vocation HIV and recoveryoriented community services.
  • Document services in each participants record/chart in accordance with agency standards managed care insurance providers and licensing requirements. Utilize the prescribed format consisting of Data Assessment and Plan.
  • Document interactions such as phone calls phase progressions and updates from status listings in the Treatment Management Information Systems Database (TMIS).
  • Prepare graduation packets for assigned participants completing Phase IV on the Thursday before the graduation ceremony.
  • Provide monthly caseload statistics on the Friday following each monthly graduation ceremony as well as submissions for potential graduates for the following month.
  • Assist participants with enrolling in the Department of Public Assistance services.
  • Play a key role in developing intra and interorganizational linkages fostering collaborative relationships with various programmatic entities.
  • Conduct urine drug screens for Treatment Court participants.
  • Meet regularly with participants in treatment facilities and recovery houses to follow up on courtordered sanctions or requests and phase calculations.
  • With other members of the case management unit be available to participants families treatment facilities judges and legal teams Monday through Friday from 8:30 A.M. to 5:00 P.M.
  • Check work email and voicemail throughout the workday.
  • Consistently assess each assigned participants progress during individual sessions using the PCPC (Pennsylvania Client Placement Criteria) standards to ensure their progress is accurately reflected at the PCPCs level of care.
  • Perform all other duties as assigned.

  • Ability to meet timelines
  • Excellent oral and written communication skills
  • Good analytical and problemsolving skills
  • Working knowledge of DSMIV and cooccurring issues
  • Working knowledge of participants with forensic affiliation
  • Preferred: Bilingual in Spanish and English

REQUIREMENTS:

Must meet one of the following:

  • Associate degree in social services sociology psychology criminal justice or social work and 2 years of experience in human services including 1 year of direct participant contact in an addiction recovery mental health or behavioral health setting
  • Bachelors degree with major coursework in sociology social work psychology anthropology political science history criminal justice counseling or education and 1 years of experience involving direct participant contact in an addiction recovery mental health or behavioral health setting

PREFERRED EXPERIENCE:

  • Experience in case management for participants with forensic backgrounds who have abused illicit psychoactive substances
  • Experience in courtroom presentations

SALARY GRADE: 16
