About the Opportunity
POSITION SUMMARY
The PAY Case Managersupports participants aged 1221 in completing community service hours and achieving personal goals. They coordinate youth program activities and build partnerships with community organizations.
PRIMARY DUTIES AND RESPONSIBILITIES:
Duties and responsibilities of the Pay Case Manager include but are not limited to:
Administrative and Program related
- Coordinates communicates and monitors participants case plan to encourage successful completion of program
- Oversees scheduling of youth in the program;
- Documents all contacts and information as per guidelines;
- Tracks youths attendance
- Completes monthly reports;
- Liaises with community organizations to develop partnerships based on program and participant needs;
- Implements and adheres to all agency and program policies and procedures;
- Identifies and communicates concerns and updates regarding community programming;
- Attends and participates in program meetings.
- Addresses work centre concerns with appropriate personnel when necessary and in a timely manner;
- Ensures all adult volunteer mentors students and new staff receive support training and direction
- Supervises mentors while working in the community
- Provides feedback and evaluation to mentors and students
Participant Related:
- Builds positive relationships and rapport with participants helping to identify personal strengths to promote a sense of belonging independence generosity and mastery
- Acts as an advocate on behalf of the best interest of youth ensuring their rights are respected;
- Makes links and connections with participants in the community to support their skill development utilizing volunteer work and participant goals;
- Acts as a positive role model for youth;
- Ensures all participants receive support encouragement and followup.
- Manages case load including intake scheduling and follow up
QUALIFICATIONS:
The successful candidate should demonstrate the followingcompetencies which incorporate the Seven Sacred Teachings;
- Postsecondary degree in Human Services or other related field;
- Minimum of four years of related experience working with highrisk youth;
- Demonstrated experience in group facilitation;
- Experience with oneonone mentorship with youth and demonstrate an understanding of the realities of living independently;
- Ability to navigate crisis and support others through stressful situations;
- Knowledge of strengthbased philosophy and knowledge of aboriginal culture and language (preferred);
- Knowledge and experience with diverse cultures and economic backgrounds;
- Knowledge of community resources;
- Willingness to obtain necessary training as per the organizations training policy including Crisis Deescalation Applied Suicide Intervention Naloxone Colonization Awareness and Child Development;
- Satisfactory Criminal Record Check with Vulnerable Sector Search a clear Child Abuse Registry check with acceptable Prior Contact checks and a clear Adult Abuse Registry check all done within the last 3 months;
- Valid drivers license and acceptable drivers abstract (subject to approval);
- Access to a personal vehicle for work is required;
- Ability to work days evenings and weekends.

SALARY RANGE: $18.80 22.37 per hour. Depending on the qualification.
HOURS OF WORK: 37.5 hours per week combination of days evenings and weekends.
