About the role

The Harm Reduction Navigator plays a vital role in providing comprehensive case management services to clients with substance use mental health and addiction needs. You will collaborate closely with clients to support care plans facilitating access to various services such as counseling therapy and social support. In this position you will ensure seamless coordinated care by working alongside healthcare professionals community organizations and external agencies.

This position plays a key role in supporting the communitybased substance use program a phased program and focuses on engagement stabilization skills development and transition.

This position reports directly to the Supervisor Special Projects.
