About the role
The Harm Reduction Navigator plays a vital role in providing comprehensive case management services to clients with substance use mental health and addiction needs. You will collaborate closely with clients to support care plans facilitating access to various services such as counseling therapy and social support. In this position you will ensure seamless coordinated care by working alongside healthcare professionals community organizations and external agencies.
This position plays a key role in supporting the communitybased substance use program a phased program and focuses on engagement stabilization skills development and transition.
What you will be doing
This position reports directly to the Supervisor Special Projects.
- Assessment: Conduct comprehensive assessments of clients substance use mental health and addiction needs considering their unique circumstances strengths and challenges.
- Care Planning: Collaborate with clients to support services that address their substance use mental health and addiction goals. Monitor progress and adjust care plans as needed throughout the programs phases.
- Service Coordination: Facilitate access to a range of services including counseling therapy medication management and social support to meet clients needs.
- Advocacy: Advocate for clients rights and needs within the healthcare system ensuring they receive quality care and support. Assist clients in accessing transitional supports including housing stabilization employment services and community wellness programs.
- Collaboration: Work closely with other healthcare professionals community organizations and agencies to ensure seamless and coordinated care for clients. Collaborate with community partners such as AA (Alcoholics Anonymous) NA (Narcotics Anonymous) and other support services to enhance clients recovery journey.
- Documentation: Maintain accurate and uptodate client records documenting assessments care plans progress and outcomes. Provide biweekly reports to the Harm Reduction Supervisor on program outcomes and client needs.
- Cultural Competence: Demonstrate cultural sensitivity and competence recognizing the diverse backgrounds and needs of clients within the Mtis community.
- Liaison: Liaise with other MNO mental health services and support teams on cases to ensure continuity of care.
- Knowledge Sharing: Actively and independently keep uptodate on mental health and addiction services available across the province for children youth and families.
- Representation: When directed represent the MNO at community events and activities related to mental health and addictions.
- Project Work: Implement annual project work plans and activity reports in accordance with program goals.
- Record Keeping: Maintain accurate and uptodate records of all programrelated activities.
- Reporting: Provide biweekly reports to the Supervisor of Mental Health and Addictions detailing client progress program outcomes and service needs.
- Privacy and Confidentiality: Uphold strict standards of privacy and confidentiality in compliance with guidelines organizational policies and applicable privacy laws.
- Other reasonable duties as may be assigned
Qualifications
- Strong desire to act in the service of the Mtis Citizens to advance and uphold rights and culture
- Completed postsecondary degree in a related field (Social Work Addictions or Mental Health) or a minimum of 2 years of experience in mental health and addictions with a focus on harm reduction.
- Knowledge of harm reduction strategies and experience working with Indigenous or Mtis populations is an asset.
- Computer Proficiency: Competency in using Microsoft Office programs (Outlook Excel Word PowerPoint) and client health care management software to track client interactions and service delivery.
- Communication: Excellent oral and written communication skills with the ability to convey information and advocate for clients.
- Organization and Time Management: Strong organizational skills with the ability to manage multiple tasks prioritize work and meet deadlines.
- Selfmotivation: Demonstrated ability to work independently with minimal supervision as well as collaboratively in a team environment.
- Problemsolving and Advocacy: Ability to resolve client issues advocate effectively for their needs and coordinate care across multiple systems.
- Demonstrated ability to work alone/independently with minimal supervision
Preferred
- Completion of a Masters degree in a relevant field
- Knowledge of Mtisspecific and broader indigenous policy and research key challenges and programs
Experience
- Proven experience working in the field of adult mental health and addictions with a strong understanding of harm reduction approaches.
- Experience and knowledge of Indigenous issues particularly those affecting Mtis peoples are considered an asset.
- Strong knowledge of Ontarios mental health and addiction programs and services including best practices in care coordination and client advocacy.
Preferred
- Previous 2 years or more experience in a similar/comparable role
- Demonstrated ability to write high quality evidence and databased reports and professional presentations
- Demonstrated ability in policy and program development and evaluation
- Demonstrated ability to quickly acquire and expand knowledge and to be flexible in a demanding and dynamic environment
- Previous experience dealing with confidential information
