Job Description:
We are looking for a Homemakers & Health Services Nurse Registered Nurse to join our team! The individual in this key position is responsible for conducting assessments supervision and training in accordance with licensing requirements and agency policies. Additional responsibilities include but are not limited to:
- Conduct initial and ongoing nursing assessments for individuals on personal case load by conducting home health visits at client specific intervals.
- Conduct new client intake visits and complete necessary documents for admittance to our services.
- Develop approve and monitor care plans to reflect patient status with reassessment at appropriate intervals.
- Promote safety awareness for staff and individuals served.
- Assist individuals served with coordination of care medication management and other RN specific duties.
- Work in collaboration with Care Managers PCP offices Clinicians and other community agencies to meet the needs of our individuals served.
Qualifications:
- Registered Nurse license with the New Hampshire Board of Nursing required
- Five 5 years of experience working in home care preferred
- Three 3 years of supervisory experience preferred
- Strong computer organization communication and problemsolving skills
- Willingness to travel and work flexible hours to meet the needs of the program
- Valid drivers license and reliable transportation
Compensation: $35.00 hourly
Hours: This is a PartTime position offering the flexibility to meet desired weekly hours.
