The Mobile Integrated Health Community Paramedic (MIH/CP) is an integral partner in delivering the future of healthcare by providing the right care at the right time in the right place. The MIH/CP is a skilled clinician that can perform medical evaluation treatment and stabilization under close medical direction. The MIH/CP conducts both planned and ondemand urgent visits as requested by the Hospital at Home team with medical oversight and direction provided by the Hospital at Home provider. As such the role requires the ability to perform procedures independently such as initiating intravenous access phlebotomy and administering medications with oversight. The MIH/CP facilitates continuity of care by interacting with patients providers and other members of the health care team.
Responsibilities
Performs procedures and treatments within scope of practice based on policy and procedure available resources and circumstances presented by the individual patient as part of the Hospital at Home model. Review and administration of medications as ordered. Set up Remote Patient Monitoring equipment as needed. Is a good steward of HaH resources and manages time efficiently. Maintains confidentiality of patient personnel and organizational information. Demonstrates competency in utilization of departmental and organizational information systems supplies and resources related to patient care. Assumes responsibility for continuing education to maintain current knowledge base; seeks and maintains knowledge related to a variety of disease processes and related medications to Hospital at Home patient population. Ability to critically think and change patient load based on acuity and needs of the department. Communicates regularly and demonstrates professional behavior at all times in verbal and written interactions with patients families and coworkers. Performs other work duties as assigned based on the needs of Hospital at Home department. Performs other duties as assigned.
Qualifications
EDUCATION:
Required: High School Diploma
EXPERIENCE:
Required: A minimum of 3 years experience in a 911 community paramedicine or acute healthcare setting. Strong clinical/procedural skills required. Must be deemed competent by Health at Home Medical Director and the Cone Health MIH/CP onboarding and release to practice standard
LICENSURE/CERTIFICATION/REGISTRY/LISTING:
Required: Active North Carolina State Paramedic certification Must obtain CPC (certified community paramedic) certification within 18 months of hire BLS (CPRAmerican Red Cross or AHA Healthcare Provider ACLS American Red Cross or Healthcare Provider Valid drivers license and clean driving record
