Job Description

Manages and coordinates all aspects of program design and operations forMGH Senior HealthWISE a community program which provides services to lowincomeolder adults in neighborhoods surrounding MGH. Shares responsibility for the financialmanagement of the program with Administrative Manager for Geriatric Medicine.

Creates program curricula based on needs of the community sets goalsand evaluates the effectiveness of interventions.

Manages small Senior Healthwise staff.

Coordinates programs / initiatives with MGH Senior Health (a primarycare practice) as appropriate; and facilitates access to MGH Primary Care for participantsin Senior Healthwise programs.

Provides expert clinical services to patients and families.

PRINCIPAL DUTIES AND RESPONSIBILITIES

Clinical and Programmatic:

Works with lowincome housing building managers community groups andclinicians in Sr. Health to identify patient / family needs and developprograms to meet them.

Coordinates community outreach and enrollment / connection withprograms.

Provides/assures the quality of Senior HealthWISE services incollaboration with clinical leaders of MGHSenior Health.

Assures appropriate and efficient use of MGH resources.

Identifies problems/issues for quality improvement; and leads/participatesin formal quality improvement and program evaluation activities.

Participates in research projects.Writes grants assesses benchmarks prepares reports and proposals.

Assumes other duties as necessary.

Collaboration:

Collaborates with other MGH departments Partners and communitycolleagues to meet needs of target patient population and enable programs tosucceed.

Conducts multidisciplinary case review involving SeniorHealth and Senior HealthWISE staff.

Serves on committees as needed.

PRINCIPALDUTIES AND RESPONSIBILITIES (Continued)

Organizational/Administrative Skills:

Responsible for overall administration of Senior HealthWISE Programincluding: hiring orientation and training / supervision of staff;development / collection of program data; documentation of program activitiesand periodic program reports; and program evaluation.

Conducts team meetings for program and overseesoverall functioning of program staff.

Works closely with Development Office Center forCommunity Health Improvement and Public Relations.

Professional Conduct:

Adheres to the professionsclinical standards and Code of Ethics.

Promotes patient/family satisfaction with Programsexperience.

Promotes sensitivity to race class cultural andsexual diversity.

Adheres to the Department and Hospital standardspolicies and procedures.

Promotes Program Departmental and Hospital publicrelations.

Professional Development:

Consults with MGH Senior Health leadership team.

Is responsible for own professional development. Attends pertinent continuing education andprofessional activities related to job responsibilities and career goals. Keeps current with health care developmentsas pertinent to job and profession.

Maintains Massachusetts Licensure at highest level of eligibility(LICSW).

Patient Population:

Lowincome residents in communities surrounding MGH



Office setting including in lowincome housing near MGH.

SUPERVISORY RESPONSIBILITY

Supervises up to 4 employees professional and clerical.

FISCAL RESPONSIBILITY

In collaboration with Administrative Manager for Geriatric Medicine develops and monitors annual budget.

MSW LICSW

5 years experience including program management.

Leadership and supervisory experience required.


Excellent communication skills with multidisciplinary audiences andstrong writing skills.

Demonstrated ability to lead program in complex multidisciplinaryenvironment.

Skillful problemsolver.
